摘要
目的探讨多时相CT血管成像(mCTA)彩色编码图对缺血性脑卒中(IS)出血性转化(HT)的预测价值。方法回顾性收集厦门市中医院2023年1月至2025年2月收治的96例IS患者资料,全部患者均行CT平扫、mCTA检查,依据CT平扫检查结果评估HT发生情况,分为发生组(n=32)、未发生组(n=64);对比两组的常规mCTA评分、彩色编码图评分差异,分析m CTA彩色编码图对IS患者HT的预测价值。结果96例IS患者中发生HT 32例,发生率为33.33%;发生组入院时NIHSS评分、梗死面积、彩色编码图评分均较未发生组高,常规mCTA评分较未发生组低(P<0.05);绘制ROC曲线发现,常规mCTA评分、彩色编码图评分预测IS患者HT发生的AUC分别为0.863、0.947,均有预测价值,且彩色编码图评分预测价值更高,AUC>0.90。结论mCTA彩色编码图对IS患者HT具有较高的预测价值,考虑未来可作为患者HT评估重要检查方案之一。
Objective To investigate the predictive value of multi-phase CT angiography(mCTA)color-coded images for hemorrhagic transformation(HT)in ischemic stroke(IS).Methods A retrospective review was conducted of 96 IS patients admitted to Xiamen Traditional Chinese Medicine Hospital between January 2023 and February 2025.All patients underwent CT plain scanning and mCTA examinations.Patients were divided into occurrence and non-occurrence groups according to the HT occurrence assessed based on CT plain scanning results.The conventional mCTA scores and color-coded map scores between the two groups were compared to analyze the predictive value of mCTA color-coded maps for HT in IS patients.Results Among the 96 IS patients,32 developed HT,with an incidence rate of 33.33%.The occurrence group had higher NIHSS scores,infarct areas,and color-coded map scores at admission compared to the non-occurrence group,while the conventional mCTA scores were lower than those of the non-occurrence group(P<0.05).ROC curve analysis revealed that the AUC values for predicting HT occurrence in IS patients using conventional mCTA scores and color-coded map scores were 0.863 and 0.947,respectively,indicating predictive value.The color-coded map score demonstrated higher predictive value with an AUC>0.90.Conclusion Color-coded maps from mCTA have high predictive value for HT in IS patients and may serve as an important diagnostic tool for assessing HT in the future.
