摘要
朱珊教授认为血瘀是小儿过敏性紫癜的基本病机,临证擅用六经辨证治疗,并提出“祛瘀六法”。即太阳法,从太阳病辨治,以开郁启闭、消瘀通络为要,常予银翘散加减;少阳法,调治少阳枢机,行气散瘀,常予小柴胡汤加减;阳明法,注重通胃腑,清热瘀,即荡涤阳明之壅,祛热瘀之实,常予桃核承气汤合犀角地黄汤加减;少阴法,根据“阳病治阴”的原则,强调滋阴养阴,以养真阴,消虚瘀,常予六味地黄丸加减;厥阴法,从厥阴辨治寒热虚实夹杂之患,常予乌梅丸加减;太阴法,结合太阴病的发病特点,以温阳扶阳为主,少佐活血补血药,常予四物汤加减。
Professor ZHU Shan believes that blood stasis is the fundamental pathogenesis throughout pedi-atric Henoch-Schönlein purpura(HSP).In clinical practice,she is skilled in applying the Six Meridians syn-drome differentiation theory for treatment and has proposed the“Six Methods for Eliminating Stasis.”These are:the Taiyang Method,which involves differentiation and treatment from the perspective of Taiyang disease,focusing on opening depression,relieving obstruction,dissipating stasis,and unblocking collaterals,often us-ing modified Yinqiao San(银翘散);the Shaoyang Method,which regulates the Shaoyang pivot,moves qi,and disperses stasis,using modified Xiaochaihu Tang(小柴胡汤);the Yangming Method,which emphasizes un-blocking the stomach fu,clearing heat and stasis,namely,sweeping away Yangming congestion and eliminat-ing excess heat and stasis,often employing modified Taohe Chengqi Tang(桃核承气汤)combined with Xijiao Dihuang Tang(犀角地黄汤);the Shaoyin Method,which based on the principle of"treating yin for yang dis-ease,"emphasizes nourishing yin and cultivating yin to nourish true yin and dissipate deficiency stasis,often using modified Liuwei Dihuang Wan(六味地黄丸);the Jueyin Method,which involves differentiation and treatment from the Jueyin perspective for conditions characterized by a mixture of cold,heat,deficiency,and excess,often using modified Wumei Wan(乌梅丸);and the Taiyin Method,which considering the pathologi-cal characteristics of Taiyin disease,focuses on warming yang and supporting yang,supplemented with a small amount of blood-activating and blood-tonifying herbs,often using modified Siwu Tang(四物汤).
