基于奥马哈系统的个案管理在老年痴呆患者中的应用

摘要 目的 探讨基于奥马哈系统的个案管理在老年痴呆患者中的应用及对其认知功能、日常生活能力和心理状态的影响。方法 选择2023年10月—2024年8月医院收治的符合标准的60例老年痴呆患者作为研究对象,按照组间基线资料具有可比性原则将其分为对照组和观察组,各30例。对照组采用常规护理干预,观察组采用基于奥马哈系统的个案管理。对比两组患者认知功能、日常生活能力、心理状态、就医体验。结果 干预后,观察组患者简易智力状态量表(MMSE)中的定向力、计算力、记忆力、回忆力、语言能力5个维度评分均高于对照组,差异有统计学意义(P<0.05)。干预后,观察组患者日常生活能力量表(ADL)中躯体生活自理能力、工具性日常生活能力2个维度评分均低于对照组,差异有统计学意义(P<0.05)。干预后,观察组患者健康问卷(PHQ-9)、广泛性焦虑障碍量表(GAD-7)评分均低于对照组,差异有统计学意义(P<0.05)。干预后,观察组患者就医体验量表(HCAHPS)中的护患沟通、医院环境、需求应答、用药沟通、出院指导、整体评价6个维度的评分均高于对照组,差异有统计学意义(P<0.05)。结论 基于奥马哈系统的个案管理对老年痴呆患者认知功能、日常生活能力和心理状态的改善均具有促进作用,进而提升患者的就医体验。 Objective To investigate the application of case management based on the Omaha system in patients with senile dementia and the effects on cognitive function,activities of daily living,and psychological status.Methods Sixty eligible patients with dementia admitted to the hospital from October 2023 to August 2024 were selected.They were divided into a control group and an observation group(30 patients each)based on the principle of comparable baseline characteristics.The control group received routine nursing interventions,the observation group underwent case management based on the Omaha system.Cognitive function,activities of daily living,psychological status,and healthcare experience were compared between groups.Results After intervention,the observation group demonstrated statistically significantly higher scores than the control group across all five dimensions of the Mini-Mental State Examination(MMSE):orientation,calculation,memory,recall,and language ability(P<0.05).After intervention,the observation group exhibited statistically significantly lower scores than the control group in two dimensions of the Activities of Daily Living(ADL)scale:physical selfcare and instrumental activities of daily living(P<0.05).After intervention,the observation group exhibited lower scores than the control group on the Patient Health Questionnaire(PHQ-9)and Generalised Anxiety Disorder-7(GAD-7)scales,with statistically significant differences(P<0.05).After intervention,patients in the observation group demonstrated statistically significantly higher scores than the control group across six dimensions of the Healthcare Consumer Assessment of Hospital Experience Survey(HCAHPS):nurse-patient communication,hospital environment,responsiveness to needs,medication communication,discharge instructions,and general evaluation(P<0.05).Conclusion Case management based on the Omaha System promotes improvements in cognitive function,activities of daily living,and psychological status among patients with dementia,thereby enhancing their healthcare experience.
