摘要
目的研究旨在探究血细胞分析中性粒细胞-淋巴细胞比率(NLR)、血小板-淋巴细胞比率(PLR)、淋巴细胞-单核细胞比率(LMR)和淋巴细胞-C反应蛋白比率(LCR)与青少年腺病毒(ADV)感染的相关性及预测价值。方法选取2024年1-9月本院儿科收治的110例腺病毒核酸阳性患儿作为观察组,另选取了同期健康体检儿童147例作为对照组,通过血细胞分析参数及C反应蛋白水平计算NLR、PLR、LMR以及LCR。采用R软件,Mann-Whitney U检验比较两组资料的差异,以及受试者工作特征(ROC)曲线来评估NLR、PLR、LMR以及LCR对腺病毒感染的预测价值,并根据公共数据库外周血转录组数据分析其潜在的科学性。结果与对照组相比,腺病毒感染患儿的NLR、PLR、LMR以及LCR差异均有统计学意义(均P<0.05)。在ROC中NLR、PLR、LMR以及LCR的最佳截断值分别为1.956、119.693、4.343及0.499。曲线下面积(AUC)最大的指标为LCR,特异度为0.897。NLR的灵敏度最高,为0.884。结论NLR、PLR、LMR以及LCR在青少年腺病毒感染中显示出良好的预测价值,可以为基层医疗机构腺病毒感染诊断提供依据。
Objective To explore the correlation and predictive value of neutrophil lymphocyte ratio(NLR),platelet lymphocyte ratio(PLR),lymphocyte monocyte ratio(LMR),and lymphocyte-C reactive protein ratio(LCR)in blood cell analysis with adolescent adenovirus(ADV)infection.Methods A total of 110 adenovirus nucleic acid positive children admitted to the hospital from January to September 2024 were selected as the observation group,and 147 healthy children who underwent physical examinations during the same period were selected as the control group.NLR,PLR,LMR,and LCR were calculated based on blood cell analysis parameters and C-reactive protein levels.Differences between groups were analyzed using the Mann-Whitney U test in R software.Receiver operating characteristic(ROC)curves were constructed to evaluate the predictive performance of NLR,PLR,LMR,and LCR for ADV infection.Public transcriptomic datasets of peripheral blood were further used to explore the biological plausibility of these markers.Results Compared with the control group,children with ADV infection showed significant differences in NLR,PLR,LMR,and LCR(all P<0.05).In ROC curve analysis,the optimal cutoff values for NLR,PLR,LMR,and LCR were 1.956,119.693,4.343,and 0.499,respectively.LCR demonstrated the highest area under the curve(AUC)with a specificity of 0.897,whereas NLR exhibited the greatest sensitivity(0.884).Conclusion NLR,PLR,LMR,and LCR have shown good predictive value in adolescent adenovirus infections and can provide a basis for the diagnosis of adenovirus infections in primary healthcare institutions.
