摘要
目的:探讨高分辨率磁共振血管壁成像(HR-VWI)对颈动脉闭塞(CAO)介入开通的术前评估价值。方法:选取2021年10月至2022年10月我院拟接受介入治疗的30例CAO患者为研究对象,术前均完善HR-VWI,以术中数字减影血管造影(DSA)检查为金标准,评估HR-VWI的准确性。结果:HR-VWI结果显示,完全闭塞CAO患者21例,次全闭塞9例,与DSA比较(完全闭塞23例,次全闭塞7例),诊断符合率为93.33%,Kappa值为0.831;Ⅰ、Ⅱ、Ⅲ级闭塞分别为7例、18例和5例,与DSA比较(Ⅰ、Ⅱ、Ⅲ级闭塞分别为7例、20例和3例),诊断符合率为93.33%,Kappa值为0.874;A、B、C、D型闭塞分别为15例、6例、4例和5例,与DSA比较(A、B、C、D型闭塞分别为15例、6例、3例和6例),诊断符合率为90.00%,Kappa值为0.849。结论:HR-VWI作为一种非侵入性影像学评估手段,在CAO患者介入开通治疗的术前评估中展现出良好的有效性与可靠性。
Objective:To evaluate the preoperative assessment value of high-resolution magnetic resonance vessel wall imaging(HR-VWI)for interventional recanalization in carotid artery occlusion(CAO).Methods:From October 2021 to October 2022,30 CAO patients scheduled for interventional therapy underwent preoperative HR-VWI.Using intraoperative digital subtraction angiography(DSA)as the gold standard,the diagnostic accuracy of HR-VWI was assessed.Results:HR-VWI identified 21 complete occlusions and 9 subtotal occlusions,showing 93.33% consistency(Kappa=0.831)with DSA findings(23 complete occlusions and 7 subtotal occlusions).For occlusion grading,HR-VWI classified 7,18,and 5 cases as grades Ⅰ,Ⅱ,and Ⅲ,respectively,achieving93.33% consistency(Kappa=0.874)with DSA(7,20,and 3 cases).For occlusion types,HR-VWI detected 15,6,4,and 5 cases of types A,B,C,and D,demonstrating 90.00% consistency(Kappa=0.849)with DSA(15,6,3,and 6 cases).Conclusion:As a non-invasive imaging assessment method,HR-VWI demonstrates substantial validity and reliability in preoperative evaluation for CAO interventional recanalization.
