摘要
目的探究疏肝清热方对微波消融术改善浆细胞性乳腺炎(PCM)中辅助性T细胞(Th)1/Th2细胞平衡的影响。方法将PCM患者随机分为对照组和试验组。对照组给予微波消融术治疗,试验组给予疏肝清热方联合微波消融术治疗。比较治疗前后2组患者乳房疼痛VAS评分和临床疗效;检测2组患者血清免疫球蛋白、补体水平及Th1/Th2型细胞因子水平。将小鼠随机分为对照组、PCM组、微波消融术(MWA)组、疏肝清热方(SGQRD)组和疏肝清热方联合微波消融术(SGQRD+MWA)组,给予相应处理,检测免疫球蛋白、补体、Th1/Th2型细胞因子水平及乳腺组织病理损伤情况。结果与对照组相比,试验组患者乳房疼痛减轻,血清IgA、IgG、IgM、C3、C4、γ干扰素(IFN-γ)、白细胞介素(IL)-2、肿瘤坏死因子α(TNF-α)水平降低,IL-4、IL-5、IL-13水平升高,治疗总有效率提高(P<0.05)。与对照组相比,PCM组小鼠乳腺结构紊乱,存在大量炎症细胞浸润,血清IgA、IgG、IgM、C3、C4水平升高,血清和乳腺组织IFN-γ、IL-2、TNF-α水平升高,IL-4、IL-5、IL-13水平降低(P<0.05);与PCM组相比,MWA组、SGQRD组、SGQRD+MWA组小鼠乳腺结构趋于正常,可见少量炎症细胞浸润,血清IgA、IgG、IgM、C3、C4水平降低,血清和乳腺组织IFN-γ、IL-2、TNF-α水平降低,IL-4、IL-5、IL-13水平升高(P<0.05);且SGQRD+MWA组的治疗效果优于MWA组、SGQRD组(P<0.05)。结论疏肝清热方能增强微波消融术对PCM中Th1/Th2细胞平衡的改善作用。
Objective To explore the effects of Shugan qingre decoction(SGQRD)combined with microwave ablation(MWA)to enhance the balance of helper T cells(Th)1/Th2 in plasma cell mastitis(PCM).Methods Patients with PCM were randomly divided into control and test groups.The control group was treated with MWA,whereas the test group was treated with SGQRD combined with MWA.The visual analog scale score and clinical efficacy of breast pain were compared between the two groups before and after treatment.Additionally,serum levels of immunoglobulins(IgA,IgG,and IgM),complements(C3 and C4),and Th1/Th2 cytokines(interferon-γ[IFN-γ],interleukin-2[IL-2],tumor necrosis factorα[TNF-α],IL-4,IL-5,and IL-13)were measured.Mice were randomly divided into control,PCM,MWA,SGQRD,and SGQRD+MWA groups.Following treatment with MWA and SGQRD,immunoglobulin,complement,and Th1/Th2 cytokine levels,and pathological alterations in breast tissue were detected.Results Compared with the control group,the test group demonstrated reduced breast pain,lower serum levels of IgA,IgG,IgM,C3,C4,IFN-γ,IL-2,and TNF-α,higher levels of IL-4,IL-5,and IL-13,and increased treatment effectiveness(P<0.05).Compared with the control group,the breast tissue of mice in the PCM group exhibited a disordered structure and significant inflammatory cell infiltration.This was accompanied by increased serum levels of IgA,IgG,IgM,C3,C4,IFN-γ,IL-2,and TNF-α,increased IFN-γ,IL-2,and TNF-αlevels in serum and breast tissue,and decreased IL-4,IL-5,and IL-13 levels(P<0.05).Compared with the PCM group,the breast tissue of mice in the MWA,SGQRD,and SGQRD+MWA groups appeared normal,with mild inflammatory cell infiltration.Serum IgA,IgG,IgM,C3,and C4 levels were decreased,whereas IFN-γ,IL-2,and TNF-αlevels in the serum and breast tissue were reduced.In contrast,IL-4,IL-5,and IL-13 levels were increased(P<0.05).Additionally,the treatment effect in the SGQRD+MWA group was superior to that in the MWA and SGQRD groups(P<0.05).Conclusion SGQRD can enhance the effect of MWA on Th1/Th2 cell balance in PCM.
