二氧化锆全瓷冠边缘位置与牙周健康及牙周微生物菌群之间的关系

摘要 目的基于高通量测序技术,探讨二氧化锆全瓷冠边缘位置对牙周健康及牙周微生物菌群的影响,为临床修复方案选择提供依据。方法选取2021年5月至2023年5月收治的40例根管治疗后需行全冠修复的患者作为研究对象,以电脑编号奇偶数字法将其分为龈上组(n=20)和龈下组(n=20)。龈上组修复边缘位于龈上1.0 mm处,龈下组修复边缘位于龈下1.0 mm处。比较两组修复前、修复24周后的口腔健康指标及牙周微生物菌群相对含量;采用Pearson相关性分析探讨口腔健康指标与牙周微生物菌群相对含量的相关性。结果修复前,两组的口腔健康指标比较,差异无统计学意义(P>0.05);修复24周后,两组的口腔健康指标均高于修复前,但龈上组低于龈下组(P<0.05)。修复前,两组的各项牙周微生物菌群相对含量比较,差异无统计学意义(P>0.05);修复24周后,两组的各项牙周微生物菌群相对含量少于修复前,且龈上组少于龈下组(P<0.05)。Pearson相关性分析显示,二氧化锆全瓷冠修复术患者的口腔健康指标与牙周微生物菌群相对含量呈显著正相关(P<0.05)。结论二氧化锆全瓷冠修复边缘位于龈上边缘对患者术后早期牙周健康的影响更小,且更有利于改善牙周微生物菌群含量。 Objective To explore the effects of zirconia all-ceramic crown edge position on periodontal health and periodontal microbial flora based on high-throughput sequencing technology,and to provide a basis for clinical repair options.Methods From May 2021 to May 2023,40 patients who needed full crown restoration after root canal treatment were selected as the research objects.The patients were divided into supragingival group(n=20)and subgingival group(n=20)by computer numbering odd-even number method.The repair edge of the supragingival group was located at 1.0 mm above the gingiva,and the repair edge of the subgingival group was located at 1.0 mm below the gingiva.The oral health indicators and the relative content of periodontal microbial flora were compared between the two groups before and after 24 weeks of repair;Pearson correlation analysis was used to explore the correlation between the oral health indicators and the relative content of periodontal microbial flora.Results Before repair,there were no significant differences in oral health indicators between the two groups(P>0.05);after 24 weeks of repair,the oral health indicators of the two groups were higher than those before repair,but those in the supragingival group were lower than the subgingival group(P<0.05).Before repair,there were no significant differences in the relative contents of periodontal microbial flora between the two groups(P>0.05);after 24 weeks of repair,the relative contents of periodontal microbial flora in the two groups were less than those before repair,and those in the supragingival group were less than the subgingival group(P<0.05).Pearson correlation analysis showed that the oral health indicators of patients with zirconia all-ceramic crown restoration were significantly positively correlated with the relative contents of periodontal microbial flora(P<0.05).Conclusion The edge of zirconia all-ceramic crown restoration on the supragingival has less impact on the early periodontal health of patients after surgery and is more conducive to improving the content of periodontal microbial flora.
