摘要
目的探讨超声右心声学造影(c-TTE)联合经颅多普勒发泡试验(c-TCD)对卵圆孔未闭(PFO)的诊断效能。方法选取2022年8月—2024年8月本院收治的PFO患者155例作为PFO阳性组,选择同期接受检查且排除了PFO的45例患者作为PFO阴性组,两组均接受c-TTE、c-TCD检查,以经食管超声心动图右心声学造影(c-TEE)或术中直接观察作为诊断金标准,对其诊断效能进行分析。结果PFO阳性组检出右向左分流(RLS)153例,占比98.71%,PFO阴性组检出2例,占比4.44%,组间差异有统计学意义(P<0.05);PFO阳性组与PFO阴性组在静息状态、Valsalva动作下的RLS分级比较,差异有统计学意义(P<0.05);RLS分级越低,PFO内径越短(P<0.05),不同RLS分级的隧道长度比较,差异无统计学意义(P>0.05);c-TTE联合c-TCD诊断PFO的特异度和灵敏度均明显高于单项检测(P<0.05)。结论c-TTE联合c-TCD用于PFO诊断,具有较高的特异度与敏感度,且能评估RLS分级,为临床诊疗提供参考依据。
Objective To investigate the diagnostic efficacy of right heart acoustic contrast echocardiography(c-TTE)combined with transcranial Doppler foam test(c-TCD)for patent foramen ovale(PFO).Methods 155 patients with patent foramen ovale admitted to our hospital from August 2022 to August 2024 were selected as the PFO positive group,and 45 patients who underwent examination and excluded PFO during the same period were selected as the PFO negative group.All patients underwent c-TTE and c-TCD examinations,and the diagnostic gold standard was right heart echocardiogram(c-TEE)or intraoperative direct observation.The diagnostic efficacy was analyzed.Results In the PFO-positive group,153 cases of right-to-left shunt(RLS)were detected,accounting for 98.71%,while in the PFO-negative group,2 cases were detected,accounting for 4.44%.The difference between the groups was statistically significant(P<0.05).When comparing RLS grading between the PFO-positive and PFO-negative groups under resting conditions and during the Valsalva maneuver,the difference was statistically significant(P<0.05).A lower RLS grade was associated with a shorter PFO diameter(P<0.05),but there was no statistically significant difference in tunnel length across different RLS grades(P>0.05).The specificity and sensitivity of combining c-TTE with c-TCD for PFO diagnosis were significantly higher than those of individual tests(P<0.05).Conclusion The combination of c-TTE and c-TCD has high specificity and sensitivity in the diagnosis of PFO,and can also evaluate RLS grading,providing reference for clinical diagnosis and treatment.
