摘要
目的在冻结肩患者接受臂丛麻醉并结合超声引导进行手法松解治疗的过程中,分析辅以经皮内热式骨骼肌附着点松解术治疗的效果,以及对康复锻炼的影响。方法筛选2024年1—12月的冻结肩患者,用数字奇偶法将64例入选者分组,超声引导臂丛麻醉下手法松解治疗予以对照组(32例),并行药物、肩关节锻炼治疗,研究组(32例)施以内热式经皮骨骼肌附着点松解术治疗,比较组间治疗效果。结果研究组的治疗有效率(93.75%)相较于对照组(75.00%)要高(P<0.05),组间疼痛、肩关节功能评分治疗前差异无统计学意义(P>0.05),治疗后,研究组术后90 d的疼痛评分(0.91±0.21)分较对照组明显要低,肩关节功能评分(92.71±9.52)分较对照组明显要高(P<0.05)。组间肩关节活动度治疗前差异无统计学意义(P>0.05),治疗后,研究组的肩关节活动度均比对照组要大(P<0.05)。研究组的并发症发生率(3.13%)较对照组(18.75%)更低(P<0.05)。结论在超声引导下实施臂丛麻醉后对冻结肩患者进行手法松解治疗的基础上,联合应用内热式经皮骨骼肌附着点松解术,具有确切的治疗效果,能够减轻患者的疼痛症状,改善肩关节功能,且具备较高的安全性,具有推广应用价值。
Objective To analyze the effect of assisted percutaneous thermal release of skeletal muscle attachment points and its impact on rehabilitation exercise in patients with frozen shoulder undergoing brachial plexus anesthesia combined with ultrasound-guided manual release therapy.Methods Frozen shoulder patients were screened from January 2024 to December 2024,and 64 selected individuals were divided into two groups using the digital parity method.The control group(32 cases)received manual release therapy under ultrasound guided brachial plexus anesthesia,and medication and shoulder joint exercise therapy were performed in parallel.Based on this,the study group(32 cases)received internal heat percutaneous skeletal muscle attachment point release surgery,and the treatment effects were compared between groups.Results The treatment efficacy rate of the study group(93.75%)was higher than that of the control group(75.00%)(P<0.05).The pain and shoulder joint function scores between the groups were P>0.05 before treatment.After treatment,the pain score(0.91±0.21)of the study group at 90 days after surgery was significantly lower than that of the control group,and the shoulder joint function score(92.71±9.52)was significantly higher than that of the control group(P<0.05).The shoulder joint range of motion between groups before treatment was P>0.05,and after treatment,the shoulder joint range of motion in the study group was higher than that in the control group(P<0.05).The incidence of complications in the study group(3.13%)was lower than that in the control group(18.75%)(P<0.05).Conclusion On the basis of manual release treatment for frozen shoulder patients under ultrasound guidance after brachial plexus anesthesia,the addition of internal heat percutaneous skeletal muscle attachment point release surgery has definite therapeutic effects,can reduce pain,improve shoulder joint function,and has high safety,which has promotional significance.
