摘要
目的:探讨超声引导液压扩张手法松解(Ultrasonic guide and manipulation under dynamic arthrography,UGMDA)治疗冻结肩的疗效及其影响因素。方法:将79例冻结肩患者随机分治疗组(48例)和对照组(31例),分别用UGMDA和药物治疗,观察前者注入液量、粘连松解程度、肱二头肌肉肌长头健滑囊显示情况与疗效的关系,并随访6个月,统计分析两组的疗效优良率。结果:两组疗效优良率分别为83.64%和29.41%,两者比较差异有统计学意义(P<0.05);治疗组注入液量≤15 mL、15～50 mL、≥50mL组,其疗效优良率分别为25.00%、86.36%、100.00%,三者间比较差异有统计学意义(P<0.05);肱二头肌长头腱滑囊不显示和显示组其疗效优良率分别为60.00%和91.49%,两者比较差异有统计学意义(P<0.05)。结论:UGMDA是诊断和治疗冻结肩的有效方法,具有创伤小、简单易行、并发症少、疗效可靠等优点;术中注入的液体量及二头肌长头腱滑囊的显影情况与关节粘连的松解效果相关。
OBJECTIVE ： To investigate the dependablity of T value in bone mineral density and CT values on body of lumber vertebre in patients with postmenopausal osteoporosis. Methods ： 124 cases of women with menopause above 2 years were choosed for the experiment and checked by double-energy bone density machine. They were divided by detection results into three groups, group A （ osteoporosis, 67 cases） ,group B （osteopenia,32 cases） and group C （normal,25 cases）. T values of cortical bone and spongiosa in the cross section of 13 centrum for all were measured, and then in three groups the dependablity of T value in bone mineral density and CT values on body of lumber vertebre were analyzed statistically. Results ： T values of cortical bone and spongiosa in three groups were respectively 287.82 ± 96. 35,296.06 ± 92.14,316.34 ± 81.18,5.72 ± 35.64 and 99.76 ±23.10,189.09 ± 37.03. There had no statistical significance on T values of cortical bone （P 〉 0.05） in three groups, but had on T values of spongiosa （ P 〈 0.05 ）. There had no dependablity of T value in bone mineral density and CT values on cortical bone of 13 c entrum （r = 0. 019, t =0. 151, P 〉 0.05 ）, but had on that with CT values of spongiosa （ r = 0. 914, t = 18. 127, P 〈 0. 05 ）. Conclusion： The detection on CT values of spongiosa in body of lumber vertebre had reference value for diagnosis of postmenopausal osteoporosis, but had not to that of cortical bone.
