摘要
化疗相关周围神经病变(Chemotherapy-Induced Peripheral Neuropathy,CIPN)是化疗患者的常见并发症之一,表现为肢体麻木、疼痛及功能障碍等。此类并发症的发生极大影响患者的生活质量,可降低化疗耐受性,甚至造成少数患者化疗中断或导致病情恶化。关于该并发症的治疗,西医药物疗效有限且副作用多,而中医药疗法具有独特优势。近些年,有大量学者开展对CIPN中医治疗及防控措施的研究。本文对CIPN的中医治疗临床研究进展进行概述,指出CIPN核心病机为“虚、瘀、寒、毒”,辨证分型包括营卫不和证、气虚血瘀证、寒凝血瘀证等。内治法主用黄芪桂枝五物汤、补阳还五汤、当归四逆汤等益气活血,外治法辅以针灸、熏洗等协同增效。
It is one of the common complications of patients treated with chemotherapy,manifested as limb numbness,pain and dysfunction.The occurrence of such complications has a great impact on the quality of life of patients,can reduce the tolerance of chemotherapy,and even cause a few patients to interrupt chemotherapy or cause deterioration of the disease.As for the treatment of this complication,Western Medicine has limited curative effect and many side effects,while Traditional Chinese Medicine has unique advantages.In recent years,a large number of scholars have carried out research on Traditional Chinese Medicine treatment and prevention and control measures of CIPN.This paper summarizes the clinical research progress of CIPN in the treatment of Traditional Chinese Medicine,and points out that the core pathogenesis of CIPN is“deficiency,blood stasis,cold and toxin”.The syndrome differentiation types include disharmony between Ying and Wei syndrome,Qi deficiency and blood stasis syndrome,cold coagulation and blood stasis syndrome,etc.The internal therapy mainly uses Huangqi Guizhi Wuwu Decoction,Buyang Huanwu Decoction,Danggui Sini Decoction,etc.the external therapy is supplemented by acupuncture,fumigation,etc.
