摘要
目的探讨胰激肽原酶联合和血明目片对糖尿病视网膜病变患者眼底情况、眼部动脉血流动力学的影响。方法选取2023年7月—2024年12月皖北煤电集团总医院内分泌科收治的120例糖尿病视网膜病变患者,按不同的治疗方法分为对照组和观察组,每组60例,分别选取患眼64只和66只。对照组口服和血明目片治疗,观察组在对照组基础上联合胰激肽原酶治疗。对比两组总有效率、眼底情况、血浆黏度、血流动力学、不良反应。结果观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组出血灶面积、黄斑厚度、血管瘤体积均优于对照组,差异均有统计学意义(P均<0.05);对照组高切黏度、低切黏度均高于观察组,差异均有统计学意义(P均<0.05);观察组眼动脉与视网膜中央动脉的收缩期峰值流速和舒张末期流速均高于对照组,阻力指数低于对照组,差异均有统计学意义(P均<0.05)。两组血浆黏度、不良反应总发生率比较,差异均无统计学意义(P均>0.05)。结论胰激肤原酶联合和血明目片可有效降低糖尿病视网膜病变患者的血液黏度,改善眼底情况与眼部动脉血流动力学,有效性与安全性高,效果优于和血明目片单一用药。
Objective To investigate the effect of pancreatic kininogenase combined with Hexue Mingmu Tablets on fundus and ocular arterial hemodynamics in patients with diabetic retinopathy.Methods A total of 120 patients with diabetic retinopathy admitted to the Department of Endocrinology,General Hospital of Wanbei Coal Power Group from July 2023 to December 2024 were selected and divided into control group and observation group according to different treatment methods,with 60 cases in each group.64 and 66 affected eyes were selected respectively.The control group was treated with Hexue Mingmu Tablets orally,and the observation group was treated with pancreatic kininogenase on the basis of the control group.The total effective rate,fundus condition,plasma viscosity,hemodynamics and adverse reactions were compared between the two groups.Results The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the observation group had better hemorrhage area,macular thickness,and hemangioma volume compared to the control group,the differences were statistically significant(all P<0.05);the levels of high shear viscosity and low shear viscosity in the control group were higher than those in the observation group,the differences were statistically significant(both P<0.05);the peak systolic velocity and end diastolic velocity of ophthalmic artery and central retinal artery in the observation group were higher than those in the control group,and the resistance index was lower than that in the control group,the differences were statistically significant(all P<0.05).There was no significant difference in the plasma viscosity and total incidence of adverse reactions between the two groups(both P>0.05).Conclusion Pancreatic kininogenase combined with Hexue Mingmu Tablets can effectively reduce the blood viscosity of diabetic retinopathy patients,improve the fundus condition and ocular arterial hemodynamics,with high effectiveness and safety,and the effect is better than that of Hexue Mingmu Tablets alone.
