摘要
目的:分析加味盆痛灵方灌肠治疗子宫内膜异位症疼痛及对血清Ca^(2+)、TRPV1水平的影响。方法:将78例符合纳排标准的湿热瘀阻证子宫内膜异位症疼痛患者随机分为观察组和对照组,每组各39例。评价两组患者的临床疗效及症状改善情况,同时检测血清中CA125、Ca^(2+)、TRPV1的水平变化。结果:(1)观察组临床总有效率94.3%,显著高于对照组53.3%(P<0.05);(2)两组治疗后VAS评分、COX痛经症状评分、生活质量评分以及焦虑量表(HAMA)和抑郁量表(HAMD)评分均有显著改善(P<0.05),且观察组改善程度较对照组显著(P<0.05)。(3)两组患者治疗前后血清CA125水平无明显改变(P>0.05);治疗后,两组患者血清Ca^(2+)水平明显升高,TRPV1水平明显降低(P<0.05),且观察组Ca^(2+)水平高于对照组(P<0.05),TRPV1水平低于对照组(P<0.05)。结论:加味盆痛灵方可明显改善湿热瘀阻型子宫内膜异位症患者的临床症状及生活质量,有较好的镇痛作用,其作用机理可能与调节血清Ca^(2+)、TRPV1水平相关。
Objective:Analyze the effect of Jiawei Pentongling formula(JWPTL)enema on the pain of endometriosis and its impact on serum Ca^(2+)and TRPV1 levels.Methods:78 patients with endometriosis pain of damp heat and blood stasis syndrome who met the inclusion and exclusion criteria were randomly divided into a treatment group and a control group,with 39 cases in each group.Evaluate the clinical efficacy and symptom improvement of two groups of patients,and detect changes in serum levels of CA125,Ca^(2+),and TRPV1.Results:(1)The total clinical effective rate of the treatment group was 94.3%,significantly higher than the control group's 53.3%(P<0.05);(2)After treatment,both groups showed significant improvements in VAS scores,COX dysmenorrhea symptom scores,quality of life scores,as well as anxiety scale(HAMA)and depression scale(HAMD)scores,with the treatment group showing a more significant improvement compared to the control group(P<0.05).(3)Serum CA125 levels showed no significant change before and after treatment in either group(P>0.05).After treatment,serum Ca^(2+)levels significantly increased in both groups,whereas TRPV1 levels significantly decreased(P<0.05).Moreover,the Ca^(2+)levels in the treatment group were higher than those in the control group(P<0.05),while the TRPV1 levels were lower than those in the control group(P<0.05).Conclusion:The JWPTL Formula can significantly alleviate the clinical symptoms and enhance the quality of life for patients with endometriosis of damp heat and blood stasis type,exhibiting a pronounced analgesic effect.Its mechanism of action may be associated with the regulation of serum Ca^(2+)and TRPV1 levels.
