摘要
目的探讨腹腔镜手术联合地诺孕素片和屈螺酮炔雌醇片治疗卵巢型子宫内膜异位症患者的效果。方法回顾性分析2020年1月至2024年8月上海市松江区妇幼保健院220例卵巢型子宫内膜异位症腹腔镜术后患者的临床资料,根据术后治疗方法分为观察组(88例)、对照组(132例)。观察组术后给予地诺孕素片联合屈螺酮炔雌醇片,对照组术后给予曲普瑞林联合屈螺酮炔雌醇片,比较两组抗米勒管激素(AMH)、卵泡刺激素(FSH)、窦卵泡计数(AFC)、血清糖类抗原125(CA125)、盆腔疼痛[视觉模拟评分法(VAS)]。结果治疗前两组AMH、FSH、AFC比较无明显差异(P>0.05);治疗后,观察组AMH、AFC高于对照组,FSH低于对照组(P<0.05);观察组治疗后AMH、FSH、AFC与治疗前比较有差异(P<0.05);对照组治疗前后AMH、FSH、AFC比较无明显差异(P>0.05)。两组治疗前CA125水平、VAS评分比较无明显差异(P>0.05);观察组治疗后CA125水平、VAS评分低于对照组(P<0.05);治疗后两组CA125水平、VAS评分与治疗前比较均降低(P<0.05)。结论腹腔镜手术联合地诺孕素片和屈螺酮炔雌醇片治疗卵巢型子宫内膜异位症患者的效果较好,能改善卵巢储备功能。
Objective To investigate the effect of laparoscopic surgery combined with Dienogest Tablets and Drospirenone and Ethinylestradiol Tablets in the treatment of patients with ovarian endometriosis.Methods The clinical data of 220 patients with ovarian endometriosis after laparoscopic surgery in Shanghai Songjiang District Maternal and Child Health Hospital from January 2020 to August 2024 were retrospectively analyzed.According to the postoperative treatment methods,they were divided into observation group(88 cases)and control group(132 cases).The observation group was given Dienogest Tablets combined with Drospirenone and Ethinylestradiol Tablets after operation,and the control group was given Triprorelin combined with Drospirenone and Ethinylestradiol Tablets after operation.Anti-Müllerian hormone(AMH),follicle stimulating hormone(FSH),antral follicle count(AFC),serum carbohydrate antigen(CA125),pelvic pain(visual analogue scale[VAS])were compared between the two groups.Results There were no significant differences in AMH,FSH and AFC between the two groups before treatment(P>0.05).After treatment,AMH and AFC in the observation group were higher than those in the control group,and FSH was lower than that in the control group(P<0.05).In the observation group,AMH,FSH and AFC after treatment were significantly different from those before treatment(P<0.05).There were no significant differences in AMH,FSH and AFC in the control group before and after treatment(P>0.05).There were no significant differences in CA125 level and VAS score between the two groups before treatment(P>0.05).After treatment,the CA125 level and VAS score in the observation group were lower than those in the control group(P<0.05).The CA125 level and VAS score after treatment were lower than those before treatment in the two groups(P<0.05).Conclusion Laparoscopic surgery combined with Dienogestin Tablets and Drospirenone and Ethinylestradiol Tablets is effective in the treatment of patients with ovarian endometriosis,which can improve ovari an reserve function.
