摘要
目的 探究三法三穴推拿手法对坐骨神经损伤(SNI)大鼠脊髓PI3 K/Akt信号通路及HIF-1α、VEGF的影响。方法 将雄性SD大鼠随机分为正常组、假手术组、模型组、推拿组,每组9只。模型组、推拿组通过使用特制手术钳对坐骨神经进行夹持法建立SNI模型,假手术组仅暴露坐骨神经、不夹持,正常组不予任何处理。术后7d,通过智能推拿手法模拟仪进行干预,模拟点、拨、揉法,分别作用于殷门穴、承山穴、阳陵泉穴;每穴/法/1min;假手术组、模型组予以抓握束缚;共干预20次,1次/d;正常组不予任何干预。于造模前、干预10d、20d后进行大鼠脊髓损伤评分(BBB评分)、斜板试验评价肌力变化。干预20次后处死大鼠,取脊髓L4~L6节段,采用Western Blot法检测脊髓腹角中PI3K、p-PI3K及HIF-1α的表达;采用ELIS A检测VEGF的表达。结果 干预10次、20次后,与正常组比较,假手术组BBB评分、斜板试验角度差异无统计学意义(P>0.05),与假手术组比较,模型组、推拿组的BBB评分、斜板试验角度均较低(P<0.01),与模型组比较,推拿组BBB评分、斜板试验的角度较高(P<0.01);与正常组、假手术组比较,干预20次后,模型组、推拿组p-PI3K蛋白的表达降低(P<0.01),推拿组高于模型组(P<0.01);与正常组、假手术组比较,模型组HIF-1α及VEGF蛋白的表达量升高,推拿组高于模型组(P<0.01)。结论 “三法三穴”推拿手法可改善SNI大鼠脊髓腹角p-PI3K、HIF-1α及VEGF的表达量,通过激活PI3K/Akt信号通路上调HIF-1α及VEGF促进血管新生相关,通过恢复脊髓受损部位的微循环恢复运动功能。
Objective To investigate the effects of“three methods and three points”of massage on PI3K/Akt signaling pathway,HIF-1α and VEGF in spinal cord of rats with sciatic nerve injury(SNI).Methods Male SD rats were randomly divided into normal group,sham operation group,model group,and massage group with 9 rats in each group.Model group and massage group established SNI model by clamping sciatic nerve with special surgical forceps,sham operation group only exposed sciatic nerve without clamping,and normal group did not receive any treatment.Seven days after the operation,the intervention was carried out by the intelligent manipulation simulator,simulating point,pulling and kneading methods,respectively acting on Yinmen point,Chengshan point,and Yanglingquan point.Each point/method/1 min;sham operation group and model group were held and bound;a total of 20 interventions,once/day;the normal group did not receive any intervention.Basso-Beattie-Bresnahan(BBB)score and inclined plate test were performed to evaluate the changes of muscle strength before and after 10 and 20 interventions.After 20 days of intervention,the rats were sacrificed,and the spinal cord segments L4~L6 were taken,and the expressions of PI3K,p-PI3K and HIF-1α in the abdominal horn of the spinal cord were detected by Western Blot.The expression of VEGF was detected by ELISA.Results After 10 and 20 interventions,there was no significant difference in BBB score and Angle of inclined plate test between normal group and sham operation group(P>0.05);compared with sham operation group,BBB score and Angle of inclined plate test in model group and massage group were lower(P<0.01),and those in massage group were higher than those in model group(P<0.01),after 20 times of intervention,compared with normal group and sham operation group,the expression of P-PI3K protein in model group and massage group was lower(P<0.01),and that in massage group was higher than that in model group(P<0.01);compared with normal group and sham operation group,the expressions of HIF-1α and VEGF in model group were higher,and those in massage group were higher than those in model group(P<0.01).Conclusion“Three methods and three points”massage can improve the expression levels of p-PI3K,HIF-1α,and VEGF in the ventral horn of the spinal cord of SNI rats,up-regulate the correlation of HIF-1α and VEGF in promoting angiogenesis by activating PI3K/Akt signaling pathway,and restore motor function by restoring microcirculation in the injured part of the spinal cord.
