摘要
围绝经期干眼(PDED)作为与内分泌变化密切相关的特殊干眼亚型,其发病率呈现显著的性别和年龄相关性。值得注意的是,PDED患者往往同时存在多种眼部症状和全身症状,形成独特的“症候群”。西医认为围绝经期干眼发病与卵巢功能衰退引起的激素水平失衡密切相关,现有的激素补充治疗和人工泪液替代治疗存在局限。临床上中医药在辨证论治方面具有独特的优势,本文从“肝肾同源”的理论内涵出发,阐述围绝经期干眼的发病机制及辨证分型,提出以“肝肾同治”为核心的治疗原则和以补益肝肾为核心、重视疏肝解郁、关注养血补肝、勿忘温肾养阴的治疗策略,并倡导中西医结合治疗,旨在为临床治疗PDED提供更充分的理论支撑和实践指导。
Perimenopausal dry eye disease(PDED),as a distinct subtype of dry eye closely associated with endocrine changes,exhibits a significant correlation in incidence with gender and age.Notably,PDED patients often present with multiple ocular and systemic symptoms simultaneously,forming a unique"syndrome".Western medicine attributes the pathogenesis of PDED to hormonal imbalances caused by declining ovarian function during perimenopause,yet current treatments such as hormone replacement therapy and artificial tear substitutes have limitations.Traditional Chinese medicine(TCM),with its unique advantages in syndrome differentiation and treatment,offers a promising approach.Based on the theory of"live and kidney sharing the same origin",this review elucidates the pathogenesis and syndrome differentiation of PDED,proposes a treatment principle centered on"simultaneous regulation of the liver and kidney",and the treatment strategy of taking tonifying liver and kidney as the core,attaching importance to soothing the liver and relieving depression,focusing on nourishing blood and tonifying the liver,and not neglecting warming the kidney and nourishing Yin,and advocates the combination of traditional Chinese and Western medicine.It aims to provide more sufficient theoretical support and practical guidance for the clinical treatment of PDED.
