摘要
目的:观察益气补肺汤加减联合西药治疗肺结核(PTB)气阴两虚证的疗效以及对血清肿瘤坏死因子配体超家族成员14(TNFSF14)、可溶性白细胞介素-2受体(sIL-2R)水平的影响。方法:纳入2021年9月—2024年9月在郑州市第六人民医院治疗的120例PTB气阴两虚证患者,采用随机数字表法分入对照组与联合组,每组60例。对照组予抗结核西药治疗,联合组在对照组基础上使用益气补肺汤加减治疗。2组均连续治疗6个月。比较2组临床疗效、中医证候疗效、气阴两虚证评分、不良反应发生率以及血清TNFSF14、sIL-2R水平。结果:治疗6个月后,联合组临床疗效总有效率93.33%(56/60)、中医证候疗效总有效率95.00%(57/60),均高于对照组78.33%(47/60)、73.33%(44/60),差异均有统计学意义(P<0.05)。治疗3个月、6个月后,2组气阴两虚证评分均较治疗前下降,联合组气阴两虚证评分均低于同期对照组,差异均有统计学意义(P<0.05)。治疗6个月后,2组血清TNFSF14、sIL-2R水平均较治疗前下降,联合组血清TNFSF14、sIL-2R水平均低于对照组,差异均有统计学意义(P<0.05)。治疗期间,联合组不良反应发生率10.00%(6/60),对照组不良反应发生率15.00%(9/60),2组比较,差异无统计学意义(P>0.05)。结论:在抗结核西药基础上联合益气补肺汤加减治疗PTB气阴两虚证疗效确切,可有效缓解临床症状,有助于痰菌转阴、肺结核病灶吸收,以及下调血清TNFSF14、sIL-2R水平。
Objective:To observe the curative effect of the combination use of modified Yiqi Bufei Decoction and western medicine on pulmonary tuberculosis(PTB)with qi and yin deficiency syndrome and its effect on the levels of serum tumor necrosis factor ligand superfamily member 14(TNFSF14)and soluble interleukin-2 receptor(sIL-2R).Methods:A total of 120 PTB patients with qi and yin deficiency syndrome treated from September 2021 to September 2024 in The Sixth People's Hospital of Zhengzhou were included.They were divided into the control group and the combination group by random number table method,with 60 cases in each group.The control group was treated with anti-tuberculosis western medicine,and the combination group was additionally treated with modified Yiqi Bufei Decoction on the basis of the treatment in the control group.Both groups were treated for six months continuously.The clinical effects,traditional Chinese medicine syndrome efficacy,scores of qi and yin deficiency syndrome,incidence of adverse reactions and levels of serum TNFSF14 and sIL-2R were compared between the two groups.Results:After six months of treatment,the total clinical effective rate was 93.33%(56/60)in the combination group,and the total effective rate of traditional Chinese medicine syndrome efficacy was 95.00%(57/60),higher than those of 78.33%(47/60)and 73.33%(44/60)in the control group,respectively,differences being significant(P<0.05).After three and six months of treatment,the scores of qi and yin deficiency syndrome in the two groups were decreased when compared with those before treatment,and the scores of qi and yin deficiency syndrome in the combination group were lower than those at the same period in the control group,differences being significant(P<0.05).After six months of treatment,the levels of serum TNFSF14 and sIL-2R in the two groups were down-regulated when compared with those before treatment,and the above two levels in the combination group were lower than those in the control group,differences being significant(P<0.05).During the treatment,the incidence of adverse reactions was 10.00%(6/60)in the combination group and 15.00%(9/60)in the control group,there being no significance in the difference(P>0.05).Conclusion:Based on anti-tuberculosis western medicine,modified Yiqi Bufei Decoction has a definite curative effect on PTB with qi and yin deficiency syndrome,which can effectively relieve the clinical symptoms,help the negative conversion of sputum and the effective absorption of PTB lesions,and down-regulate the levels of serum TNFSF14 and sIL-2R.
