紫癜性肾炎患儿血清S100A8/A9、IL-31水平及其临床意义

摘要 目的:探究紫癜性肾炎(HSPN)患儿血清S100A8/A9、白细胞介素-31(IL-31)水平及其临床意义。方法:选取96例HSPN患儿为HSPN组,按照国际小儿肾脏病研究组协会(ISKDC)分级标准分为:≤Ⅱa级12例,Ⅱb级28例,Ⅲa级31例,Ⅲb级25例。同期选择体检健康的儿童90名作为对照组,采用酶联免疫吸附法(ELISA)检测血清中S100A8/A9和IL-31的表达水平;Pearson法分析HSPN病人血清中S100A8/A9、IL-31的相关性;绘制受试者工作特征(ROC)曲线分析S100A8/A9、IL-31对HSPN的诊断价值。结果:与对照组相比,HSPN组IL-4、TNF-α表达水平显著升高(P<0.01);HSPN组血清中S100A8/A9、IL-31的表达水平与对照组相比显著升高(P<0.01);Ⅲb级HSPN病人的S100A8/A9、IL-31表达水平均显著高于≤Ⅱa级、Ⅱb级、Ⅲa级,Ⅲa级表达水平显著高于≤Ⅱa级、Ⅱb级,Ⅱb级表达水平显著高于≤Ⅱa级(P<0.05);相关性分析显示,不同肾脏病理分级HSPN患儿血清S100A8/A9和IL-31的表达水平均呈正相关(P<0.01);二者联合诊断HSPN的AUC高于S100A8/A9和IL-31单独诊断的AUC值(P<0.01)。结论:HSPN患儿血清中S100A8/A9和IL-31呈高表达,二者正相关,联合对HSPN诊断具有临床价值。 Objective:To investigate the levels and clinical significance of serum S100A8/A9 and interleukin-31(IL-31)in children with Henoch-Schönlein purpura nephritis(HSPN).Methods:A total of 96 patients with HSPN were selected as the HSPN group,according to International Study of Kidney Disease in Children(ISKDC)grade standard,12 cases were of grade≤Ⅱa,28 cases were of gradeⅡb,31 cases were of gradeⅢa,and 25 cases were of gradeⅢb.At the same time,90 healthy children were selected as the control group.The expression levels of S100A8/A9 and IL-31 in serum were detected by enzyme-linked immunosorbent assay(ELISA).Pearson method was used to analyze the correlation between S100A8/A9 and IL-31 in serum of HSPN patients,and the ROC curve was drawn to analyze the diagnostic value of S100A8/A9 and IL-31 for HSPN.Results:Compared with the control group,the expression levels of IL-4 and TNF-αin the HSPN group were obviously higher(P<0.01);the expression levels of S100A8/A9 and IL-31 in serum of the HSPN group were obviously higher than those of the control group(P<0.01);the expression levels of S100A8/A9 and IL-31 in gradeⅢb HSPN patients were obviously higher than those in grade≤Ⅱa,gradeⅡb and gradeⅢa,the expression levels in gradeⅢa were obviously higher than those in grade≤Ⅱa and gradeⅡb,and the expression levels in gradeⅡb were obviously higher than those in grade≤Ⅱa(P<0.05).Correlation analysis showed that serum S100A8/A9 and IL-31 expression levels in HSPN children with different renal pathological grades were positively correlated(P<0.01).The AUC of HSPN diagnosed by both was higher than that of S100A8/A9 and IL-31 alone(P<0.01).Conclusions:S100A8/A9 and IL-31 are highly expressed in the serum of children with HSPN,and they are positively correlated.The combination of S100A8/A9 and IL-31 has clinical value in the diagnosis of HSPN.
