摘要
目的探讨模拟子宫环境护理在新生儿黄疸光疗中的应用效果。方法选取2023年10月至2024年6月于福建省妇幼保健院接受蓝光治疗的140例高胆红素血症新生儿,采用随机数字表法分为试验组(72例,行模拟子宫环境护理)和对照组(68例,行常规护理)。观察指标包括治疗前及治疗24 h后前额、胸骨、脐上2 cm处经皮胆红素值,并调查照护者对护理工作的满意度。结果治疗后试验组前额、胸骨和腹部胆红素水平(3.11±0.17、3.56±0.14、3.13±0.11 mg/dl)均低于对照组(3.35±0.15、3.82±0.11、3.28±0.14 mg/dl)(P<0.05)。试验组照护者护理满意度达97.30%,高于对照组的75.68%(P<0.05)。结论在新生儿黄疸光疗中应用模拟子宫环境护理可更有效降低胆红素水平,并提升照护者满意度。
Objective To simulate the therapeutic effect of uterine environment nursing(bird’s nest style nursing)in the treatment of neonatal jaundice with blue light therapy.Methods A total of 140 cases of neonatal jaundice delivered in the obstetrics department of Fujian Maternal and Child Health Hospital from October 2023 to June 2024 were selected and treated with blue light therapy.The newborns were randomly divided into two groups:the control group(68 cases)received routine care for newborns,and the trial group(72 cases)received simulated uterine environment care on the basis of routine care.The treatment effect and nursing satisfaction of neonatal jaundice in the two groups were observed.Results After 24 hours of treatment,the transcutaneous bilirubin levels in the forehead,sternum,and 2cm above the navel of the trial group(3.11±0.17,3.56±0.14,3.13±0.11 mg/dl)showed a slight decrease compared to the control group(3.35±0.15,3.82±0.11,3.28±0.14 mg/dl),and the difference between the groups was statistically significant(P<0.05).The satisfaction rate of caregivers in the trial group reached 97.30%,significantly higher than the 75.68%in the control group(P<0.05).Conclusions Although simulating uterine environment nursing did not significantly improve short-term bilirubin clearance efficiency,it can significantly optimize the nursing experience by reducing therapeutic crying and reducing the risk of skin damage(satisfaction increased by 21.62%).Its safety and humanistic care value are outstanding.It is recommended to combine long-term efficacy observation(such as 72 hour bilirubin dynamic monitoring)for multi center clinical validation.
