血清CA125、CA199、HE4、AFP联合检测对卵巢癌的辅助诊断价值

摘要 目的探讨糖类抗原125(CA125)、糖类抗原199(CA199)、人附睾蛋白4(HE4)、甲胎蛋白(AFP)联合检测在卵巢癌辅助诊断中的临床价值。方法选取158例卵巢癌患者及158例卵巢良性病变患者分别设为观察组、对照组。均检测CA125、CA199、HE4、AFP水平,比较两组间各指标差异;并比较Ⅰ~Ⅱ期、Ⅲ~Ⅳ期卵巢癌患者血清各指标差异;绘制ROC曲线分析血清各指标对卵巢癌的诊断价值。结果观察组血清CA125、CA199、HE4、AFP水平分别为(65.82±5.81)U/ml、(75.63±6.38)U/ml、(188.52±11.53)pmol/L、(195.41±16.58)ng/ml,高于对照组的(25.87±2.43)U/ml、(27.89±2.48)U/ml、(72.56±6.15)pmol/L、(3.49±0.48)ng/ml,有统计学差异(P<0.05);Ⅲ~Ⅳ期卵巢癌患者血清CA125、CA199、HE4、AFP水平分别为(86.52±6.15)U/ml、(90.64±6.74)U/ml、(223.57±15.32)pmol/L、(236.65±20.54)ng/ml,均高于Ⅰ~Ⅱ期患者[(49.55±4.85)U/ml、(59.64±5.42)U/ml、(148.96±10.33)pmol/L、(152.74±14.23)ng/ml],有统计学差异(P<0.05);绘制ROC曲线显示,联合检测诊断卵巢癌的价值最高。结论联合检验血清CA125、CA199、HE4、AFP可提高卵巢癌诊断价值,利于早期识别病变,实现针对性治疗,更好改善患者预后。 Objective To explore the clinical value of combined detection of carbohydrate antigen 125(CA125),carbohydrate antigen 199(CA199),human epididymal protein 4(HE4),and alpha-fetoprotein(AFP)in the auxiliary diagnosis of ovarian cancer.Methods 158 patients with ovarian cancer and 158 patients with benign ovarian lesions were selected and set as the observation group and the control group respectively.The levels of CA125,CA199,HE4 and AFP were all detected,and the differences of each index between the 2 groups were compared.And compare the differences in various serum indicators among patients with stageⅠ~Ⅱand stageⅢ~Ⅳovarian cancer;The ROC curve was drawn to analyze the diagnostic value of each serum index for ovarian cancer.Results The levels of serum CA125,CA199,HE4 and AFP in the observation group were(65.82±5.81)U/ml,(75.63±6.38)U/ml,(188.52±11.53)pmol/L and(195.41±16.58)ng/ml respectively.It was higher than that of the control group[(25.87±2.43)U/ml,(27.89±2.48)U/ml,(72.56±6.15)pmol/L,(3.49±0.48)ng/ml],with statistically significant differences(P<0.05);The levels of serum CA125,CA199,HE4 and AFP in patients with stageⅢ~Ⅳovarian cancer were(86.52±6.15)U/ml,(90.64±6.74)U/ml,(223.57±15.32)pmol/L and(236.65±20.54)ng/ml,respectively.All were higher than those of stageⅠ~Ⅱpatients[(49.55±4.85)U/ml,(59.64±5.42)U/ml,(148.96±10.33)pmol/L,(152.74±14.23)ng/ml],with statistically significant differences(P<0.05);The ROC curve was drawn to show that the combined detection has the highest value in diagnosing ovarian cancer.Conclusion The combined test of serum CA125,CA199,HE4 and AFP can enhance the diagnostic value of ovarian cancer,facilitate the early identification of lesions,achieve targeted treatment,and better improve the prognosis of patients.
