2021—2023年某医院环境卫生学及消毒效果监测分析

Monitoring and Analysis of Environmental Hygiene and Disinfection Effects in a Hospital From 2021 to 2023

摘要 目的探讨赤峰市医院环境卫生学监测工作在医院感染防控中的作用。方法选取2021—2023年赤峰市医院环境卫生学标本16125份,包括消毒后空气(n=5819)、物体表面(n=6389)、医护人员手(n=2413)、软式内镜(n=421)、使用中消毒剂(n=398)、透析用水(n=352)、透析液(n=155)、医疗器械(n=67)、医用织物(n=62)及纯化水(n=49)等环境卫生学数据,按照GB 15982—2012《医院消毒卫生标准》对医院内空气(Ⅰ类、Ⅱ类、Ⅲ/Ⅳ类)、物体表面(Ⅰ/Ⅱ类、Ⅲ/Ⅳ类)及医护人员手(卫生手、外科手)进行分类,数据整理后进行统计学分析。结果2021—2023年采集环境卫生学标本16125份,合格15891份,合格率为98.55%。2021—2023年采集标本数分别为5628、5041、5456份,合格份数分别为5513、4970、5408份,标本合格率分别为97.96%、98.59%、99.12%,不同年份环境标本合格率比较,差异有统计学意义(P<0.001)。2021—2023年,消毒后空气、物体表面、医护人员手、软式内镜、透析用水标本合格率比较,差异有统计学意义(P<0.05)。Ⅰ类、Ⅱ类、Ⅲ/Ⅳ类环境空气合格率分别为97.96%、98.51%、98.33%;Ⅰ/Ⅱ类、Ⅲ/Ⅳ类环境物体表面合格率分别为99.27%、99.26%;卫生手消毒、外科手消毒合格率分别96.95%、97.25%。2021—2023年,Ⅲ/Ⅳ类环境空气合格率逐年提升(97.45%、98.12%、99.35%),差异有统计学意义(P<0.001);Ⅲ/Ⅳ类环境物体表面合格率分别为99.62%、98.57%、99.70%,差异有统计学意义(P<0.05)。结论清洁消毒后环境标本合格率呈上升趋势,应继续加强医院环境清洁消毒工作,为医院感染防控工作及患者医疗安全提供保障。 Objective To explore the role of environmental hygiene monitoring work in Chifeng Municipal Hospital in the prevention and control of hospital infections.Methods A total of 16125 specimens of environmental hygiene from Chifeng Municipal Hospital from 2021 to 2023 were selected,including the air after disinfection(n=5819),the surfaces of objects(n=6389),and hands of medical staff(n=2413),flexible endoscopes(n=421),disinfectants in use(n=398),dialysis water(n=352),dialysate(n=155),medical devices(n=67),medical textiles(n=62),and purified water(n=49).The air within the hospital(ClassⅠ,ClassⅡ,ClassⅢ/Ⅳ),surfaces of objects(ClassⅠ/Ⅱ,ClassⅢ/Ⅳ),and hands of medical staff(hygienic hands,surgical hands)were classified in accordance with GB 15982—2012"Hygienic Standard for Disinfection in Hospitals".After data collation,statistical analysis was conducted.Results From 2021 to 2023,a total of 16125 environmental hygiene specimens were collected,among which 15891 were qualified,with a qualification rate of 98.55%.The number of specimens collected from 2021 to 2023 was 5628,5041,and 5456 respectively.The number of qualified specimens was 5513,4970,and 5408 respectively.The qualified rates of specimens were 97.96%,98.59%,and 99.12%respectively,the qualified rate of environmental specimens in different years was compared,and the difference was statistically significant(P<0.001).From 2021 to 2023,the qualified rates of the air after disinfection,the surfaces of objects,hands of medical staff,flexible endoscopes and dialysis water samples were compared,and the difference was statistically significant(P<0.05).The qualified rates of ClassⅠ,ClassⅡand ClassⅢ/Ⅳenvironmental air were 97.96%,98.51%and 98.33%respectively.The surface qualification rates of environmental objects in ClassⅠ/Ⅱand ClassⅢ/Ⅳwere 99.27%and 99.26%respectively.The pass rates of hygienic hand disinfection and surgical hand disinfection were 96.95%and 97.25%respectively.From 2021 to 2023,the qualified rate of ClassⅢ/Ⅳenvironmental air increased year by year(97.45%,98.12%,99.35%),and the difference was statistically significant(P<0.001).The surface qualification rates of ClassⅢ/Ⅳenvironmental objects were 99.62%,98.57%,and 99.70%respectively,and the difference was statistically significant(P<0.05).Conclusion After cleaning and disinfection,the pass rate of environmental specimens has increased.We should continue to strengthen the cleaning and disinfection work of the hospital environment to provide guarantees for hospital infection prevention and control as well as ensuring the medical safety of patients.
