摘要
目的探讨计划-执行-检查-处理(PDCA)循环管理模式在医院环境卫生学监测中的应用效果。方法采用自身前后对照研究方法,以未引入PDCA循环管理模式的2017年环境卫生学监测数据作为参照,对引入PDCA循环管理模式后的2018-2020年环境卫生学监测数据的动态变化进行卡方检验或Fisher精确检验分析并对其进行两两比较。结果总体环境卫生学监测、物体表面、透析液、透析用水的合格率逐年上升,分别从2017年的95.7%、91.0%、94.5%、82.0%上升至2020年的97.5%、97.0%、100.0%、98.6%。手卫生正确率逐年上升,从2017年的88.48%上升至2020年的93.06%。2020年总体环境卫生学监测、物体表面、透析液合格率与2017年比较差异有统计学意义(P<0.05);2018-2020年透析用水合格率与2017年比较差异有统计学意义(P<0.05);2019年、2020年手卫生正确率与2017年比较差异有统计学意义(P<0.05)。卫生手消毒的合格率提高虽无统计学意义,但已达到计划目标(>85%);透析用水的合格率自2018年起已达到计划目标的合格率(>90%)。结论引入PDCA循环管理模式可有效改进医院环境卫生学监测的管理质量,从而提高医疗安全。
OBJECTIVE To investigate the effect of application of Plan-Do-Check-Act(PDCA)circle management model on hospital environmental hygiene monitoring.METHODS Chi-square test or Fisher exact test was used to analyze and compare the dynamic changes of environmental health monitoring data from 2018 to 2020 after the introduction of PDCA cycle management model,and the environmental health monitoring data in 2017 was as the reference.RESULTS The qualified rates of overall environmental hygiene monitoring,object surface,dialysate and dialysis water increased year by year,from 95.7%,91.0%,94.5%and 82.0%in 2017 to 97.5%,97.0%,100.0%and 98.6%in 2020,respectively.The accuracy rate of hand hygiene increased year by year,rising from 88.48%in 2017 to 93.06%in 2020.The differences in the qualified rates of overall environmental hygiene monitoring,object surface and dialysate between 2020 and 2017 were significantly different(P<0.05).The qualified rates of dialysis water from 2018 to 2020 was significantly different from that in 2017(P<0.05).The correct rates of hand hygiene in 2019 and 2020 was significantly different from that in 2017(P<0.05),while the qualified rate of hand disinfection was not significant,although reaching the planned target(>85%).The qualified rate of dialysis water had reached the planned target since 2018(>90%).CONCLUSION The introduction of PDCA circle management model can effectively enhance the management quality of hospital environmental hygiene monitoring,thereby improving medical safety.
