摘要
糖尿病血管内皮损伤是大血管病变早期防治的重要立足点。认为《素问·太阴阳明论篇》提出的“脾病,脉道不利”病机观与本病的“糖尿病肠道菌群紊乱-代谢产物失衡-血管内皮损伤”现代发病机制契合。提出健脾化浊利脉治法,健脾助运、实胰调肠,改善糖尿病肠道菌群紊乱;助脾散精、促精正化,减少异常代谢产物孳生;化瘀通脉、补虚复脉,恢复血管内皮损伤,协同保障脉道以通为用,为本病及其大血管病变的早期临床防治和基础研究提供思路。
Diabetes vascular endothelial injury(DVEI)is an important focus for early prevention and treatment of macrovascular diseases.The pathogenesis of"spleen disease,impeded vessel pathways"proposed in Basic Questions·Treatise on Taiyin Yangming(《素问·太阴阳明论篇》)is consistent with the modern pathogenesis of"diabetes gut microbiota dysbiosis-metabolite imbalance-vascular endothelial injury".Therefore,the therapeutic method of"fortifying the spleen,removing turbidity and promoting pulse circulation"is proposed,which is to fortify the spleen,promote transportation,strengthen the pancreas and regulate the intestine,thereby improving the intestinal flora disorder of diabetes,and can help the spleen to dissipate essence,promote the normalization of essence and reduce the breeding of abnormal metabolites.This method can also remove stasis to dredge vessel,tonify deficiency to restore vessel,restoring vascular endothelial damage,coordinating to ensure that the vessel is used for communication,and providing theoretical support and thinking direction for the early clinical prevention and treatment as well as the modern basic research of DVEI and macrovascular diseases.
