摘要
目的探讨国医名方柏苓化浊丸抗Ⅲ型慢性前列腺炎(CP)的作用机制。方法选取30只雄性SD大鼠作为研究对象,采用苯甲酸雌二醇诱导建立Ⅲ型CP大鼠模型,造模成功后随机将大鼠分为对照组、模型组、低剂量组、中剂量组、高剂量组,每组6只。给药4周取材,采用苏木精-伊红染色后观察大鼠前列腺组织形态变化,采用酶联免疫吸附试验(ELISA)法检测大鼠前列腺组织白介素(IL)-2、肿瘤坏死因子-α(TNF-α)、IL-6、IL-1β、IL-17A水平,采用Western blot法检测大鼠前列腺组织中蛋白激酶B(AKT)、磷酸化AKT(p-AKT)、磷脂酰肌醇-3激酶(PI3K)、磷酸化PI3K(p-PI3K)、核因子κB(NF-κB)p65、磷酸化NF-κB p65(p-NF-κB p65)、核因子κB抑制蛋白(IκB)、磷酸化IκB(p-IκB)的蛋白表达水平。结果与对照组比较,模型组前列腺腺体管腔整体出现萎缩、闭塞现象,可见管腔及间质周围有炎性细胞浸润成灶及许多纤维组织增生;相较于模型组,柏苓化浊丸能显著改善前列腺组织的病理变化,炎症侵染减轻。前列腺组织中IL-1β、IL-6、IL-17A、TNF-α、p-NF-κB p65、NF-κB p65、p-IκB、p-PI3K、p-AKT表达量均显著升高(P<0.05);柏苓化浊丸低、中、高剂量组中IL-1β、IL-6、IL-17A、TNF-α表达量显著降低、IL-2表达量显著升高(P<0.05)。随着给药剂量增加,p-IκB、p-PI3K、p-AKT表达量逐步降低,IκB表达量逐步升高,p-IκB/IκB、p-NF-κB p65/NF-κB p65、p-PI3K/PI3K、p-AKT/AKT比值逐步降低(P<0.05)。结论院内制剂柏苓化浊丸能一定程度上通过干预炎症因子的表达及改变p-IκB/IκB、p-NF-κB p65/NF-κB p65、p-PI3K/PI3K、p-AKT/AKT的平衡调控PI3K/AKT/NF-κB信号通路发挥抗炎作用。
Objective To explore the effect mechanism of Bailing Huazhuo Pills,a renowned traditional Chinese medicine prescription,in treating typeⅢchronic prostatitis(CP).Methods A total of thirty male SD rats were selected as the study subjects.A typeⅢCP rat model was established using estradiol benzoate induction.After successful modeling,the rats were randomly divided into a control group,model group,low-dose group,medium-dose group,and high-dose group,with six rats in each group.After four weeks of administration,tissue samples were collected.Hematoxylin-eosin staining was used to observe morphological changes in the prostate tissue.Enzyme-linked immunosorbent assay(ELISA)was employed to measure the levels of interleukin(IL)-2,tumor necrosis factor-α(TNF-α),IL-6,IL-1β,and IL-17A in the prostate tissue.Western blot was used to detect the protein expression levels of protein kinase B(AKT),phosphorylated AKT(p-AKT),phosphoinositide 3-kinase(PI3K),phosphorylated PI3K(p-PI3K),nuclear factor-κB(NF-κB)p65,phosphorylated NF-κB p65(p-NF-κB p65),inhibitor of NF-κB(IκB),and phosphorylated IκB(p-IκB)in the prostate tissue.Results Compared with the control group,the model group exhibited overall atrophy and occlusion of the prostate gland ducts,with inflammatory cell infiltration and fibrous tissue hyperplasia observed around the ducts and interstitium.Compared to the model group,Bailing Huazhuo Pills significantly improved the pathological changes in the prostate tissue,reducing inflammatory infiltration.The expression levels of IL-1β,IL-6,IL-17A,TNF-α,p-NF-κB p65,NF-κB p65,p-IκB,p-PI3K,and p-AKT in the prostate tissue were significantly elevated(P<0.05).In the low-,medium-,and high-dose groups of Bailing Huazhuo Pills,the expression levels of IL-1β,IL-6,IL-17A,and TNF-αwere significantly reduced,while the expression level of IL-2 was significantly increased(P<0.05).As the dosage increased,the expression levels of p-IκB,p-PI3K,and p-AKT gradually decreased,while the expression level of IκB gradually increased.The ratios of p-IκB/IκB,p-NF-κB p65/NF-κB p65,p-PI3K/PI3K,and p-AKT/AKT progressively decreased(P<0.05).Conclusions The in-house preparation Bailing Huazhuo Pills can exert anti-inflammatory effects by modulating the PI3K/AKT/NF-κB pathway through intervention in inflammatory factor expression and altering the balance of p-IκB/IκB,p-NF-κB p65/NF-κB p65,p-PI3K/PI3K,and p-AKT/AKT.
