摘要
红外热成像技术作为一种无创性功能影像技术,在针灸临床中的应用日益广泛。诊断方面,红外热成像技术可通过检测体表温度变化反映组织代谢及功能异常,辅助中医辨证诊断,包括寒热虚实辨证、脏腑辨证和经络辨证。治疗方面,可用于指导腧穴敏化判定、指导腧穴选择、评估针刺补泻手法效应、确定最佳艾灸参数,并客观评估多种针灸疗法的临床疗效。尽管红外热成像具有操作简便、实时动态、无辐射损伤等优势,但目前研究仍存在一些问题,如检测结果易受环境因素和患者状态影响,与穴位定位准确性密切相关;缺乏健康人群基础数据和证型判别标准;设备参数不统一且专业人才不足等。未来需要加强大样本、多中心临床研究,建立标准化诊断数据库,开发智能分析系统,完善设备技术规范,以进一步提升红外热成像在针灸临床中的应用价值,推动针灸诊疗的客观化和标准化发展。
As a non-invasive functional imaging technology,infrared thermal imaging technology is increasingly widely used in acupuncture clinic.In terms of diagnosis,infrared thermography technology can detect changes in body surface temperature to reflect tissue metabolism and functional abnormalities,and assist in the diagnosis of TCM syndrome differentiation,including Cold,Heat,deficiency and excess syndrome differentiation,Zang and Fu syndrome differentiation and meridian syndrome differentiation,and guide acupuncture and moxibustion treatment,such as acupoint sensitization determination,acupoint choosing guidance,acupuncture and laxative technique evaluation,and moxibustion parameter optimization.In terms of treatment,infrared thermography technology can be used to guide the sensitization of acupoints,guide the selection of acupoints,evaluate the effect of acupuncture and purgative techniques,determine the optimal moxibustion parameters,and objectively evaluate the clinical efficacy of various acupuncture therapies.Although IRT has the advantages of easy operation,real-time dynamics,and no radiation damage,there are still some problems,such as the detection results are easily affected by environmental factors and patient status and are closely related to the accuracy of acupoint localization.Lack of basic data and syndrome type discrimination criteria for healthy people;equipment parameters are not uniform and there is a shortage of professional personnel,etc.In the future,it is necessary to strengthen large-sample and multi-center clinical research,establish a standardized diagnostic database,develop an intelligent analysis system,and improve the technical specifications of equipment,to further enhance the application value of IRT in acupuncture clinical practice and promote the objective and standardized development of acupuncture diagnosis and treatment.
