摘要
肾系疾病以肾脏气化失司为核心病机,临床表现为水液代谢异常、精微失固及气血失调,呈现虚实夹杂的复杂证候。本文基于《素问·调经论》“气血不和,百病乃变化而生”及叶天士“久病入络”理论,提出“肾虚血瘀络损”病机理论,阐释肾虚为本、血瘀为标、络损为变的动态规律,并系统探讨4类药对在肾系疾病中的应用。文中附肾系疾病验案4则,通过典型医案佐证药对配伍的“补而不滞、攻不伤正”特点,以“肾虚血瘀络损”理论指导药对配伍,为肾系疾病防治提供新思路,尤适用于虚实错杂、迁延难愈的肾系顽疾。
Kidney diseases are characterized by the core pathogenesis of dysfunction of kidney qi transformation,clinical manifestations include abnormal water metabolism,loss of essence,and disharmony of qi and blood,presenting a complex syndrome of mixed deficiency and excess.This article proposes pathogenesis theory of“kidney deficiency,blood stasis and collateral damage”based on the theory of“disharmony between qi and blood,and the emergence of various diseases through changes”in Suwen·Tiaojinglun and“chronic diseases transforming to collaterals”in Ye Tianshi,explains dynamic pattern of kidney deficiency as the foundation,blood stasis as the standard,and collateral damage as the transformation,systematically explores the application of four types of couplet medicines in kidney diseases.Four cases of kidney diseases are attached in the article,by using typical medical cases to support the characteristic of“tonifying without stagnation,purging without harming the body”in compatibility of couplet medicines,guided by the theory of“kidney deficiency,blood stasis,and collateral damage”,provides new ideas for the prevention and treatment of kidney diseases,especially suitable for chronic kidney diseases with deficiency syndrome mingling with excess syndrome,and delayed and difficult to heal.
