摘要
喉癌是头颈部常见恶性肿瘤,其外科治疗始终面临着肿瘤根治与功能保留的双重挑战。本文综述开放性手术在喉癌治疗中的演进历程与最新进展:全喉切除术在晚期喉癌治疗中具有根治优势,但可导致永久性失声,喉移植的成功使未来根治与功能保留成为可能;部分喉切除术在早期及部分晚期喉癌中可实现肿瘤根治与功能保留,3D打印喉支架重建等技术进一步优化修复效果;个体化颈淋巴结清扫方案与精准切除技术的进步,为喉癌治疗提供更精准的手段。重点探讨其在中晚期喉癌、复杂解剖变异及挽救性治疗中的关键作用为临床决策提供循证依据。
Laryngeal cancer is a common malignant tumor of head and neck,surgical treatment of laryngeal cancer always faced with dual challenges of radical resection and functional preservation.This article reviews the evolution and latest progress of open surgery in the treatment of laryngeal cancer:total laryngectomy has a radical advantage in the treatment of advanced laryngeal cancer,but it will lead to permanent aphonia,the success of laryngeal transplantation makes it possible to cure and preserve function in the future;partial laryngectomy can achieve radical tumor resection and functional preservation in early and partial advanced laryngeal cancer,and 3D printing laryngeal stent reconstruction and other technologies further optimize repair effect;the progress of individualized neck lymph node dissection and precise resection technology provides a more accurate means in the treatment of laryngeal cancer.Focus on exploring its key role in advanced laryngeal cancer,complex anatomical variations,and salvage treatment to provide evidence-based basis for clinical decision.
