摘要
目的探讨超微创前列腺扩开术(prostatic urethral dilation,PUD)的技术改良及其在治疗小体积良性前列腺增生(benign prostatic hyperplasia,BPH)中的临床疗效与安全性。方法回顾性分析2023年6月—2024年6月在上海交通大学医学院附属新华医院接受PUD治疗的10例小体积BPH患者的临床资料。于术前、术后第1天、术后1个月及术后第6个月评估国际前列腺症状评分(International Prostate Symptom Score,IPSS)、生活质量(Quality of Life,QoL)评分、最大尿流率(maximum urinary flow rate,Qmax)、血红蛋白(hemoglobin,Hb)水平及国际勃起功能指数-5(International Index of Erectile Function-5,IIEF-5)。通过经直肠超声与膀胱镜检查观察术后膀胱黏膜完整性、出血情况及并发症。结果术后IPSS与QoL评分较术前显著下降(P<0.01),Qmax显著提升(P<0.01),IIEF-5评分较术前亦有改善(P<0.05)。术前与术后第1天Hb水平比较差异无统计学意义(P>0.05),术中出血量少。所有患者术后均未发生尿失禁。结论PUD作为一种新型超微创治疗手段,在小体积BPH患者中具有良好的安全性与显著的临床疗效,能有效改善下尿路症状,对性功能保护亦有益处,且术后尿控能力良好,无尿失禁发生,具有良好的临床应用前景。
Objective To evaluate the technical refinements of prostatic urethral dilation(PUD),a novel minimally invasive procedure,and to assess its clinical efficacy and safety in the treatment of small-volume benign prostatic hyperplasia(BPH).Methods A retrospective analysis was conducted on 10 patients with small-volume BPH who underwent PUD from Xinhua Hospital Affiliated to Shanghai JiaoTong University School of Medicine between June 2023 and June 2024.The International Prostate Symptom Score(IPSS),Quality of Life(QoL)score,maximum urinary flow rate(Qmax),hemoglobin(Hb)level,and International Index of Erectile Function-5(IIEF-5)were assessed preoperatively,on postoperative day 1,and at 1 and 6 months postoperatively.Postoperative bladder mucosal integrity,bleeding,and complications were evaluated using transrectal ultrasound and cystoscopic examination.Results Postoperative IPSS and QoL showed significant improvement compared to baseline(P<0.01).Qmax increased significantly(P<0.01),and IIEF-5 also demonstrated a postoperative improvement(P<0.05).There was no significant change in Hb levels between preoperative and postoperative day 1 measurements(P>0.05),and intraoperative bleeding was minimal.No cases of postoperative urinary incontinence were observed.Conclusion PUD is a safe and effective super minimally invasive option for the management of small-volume BPH.It significantly alleviates lower urinary tract symptoms,contributes to erectile function,and preserves postoperative urinary continence.These findings support its promising clinical utility.
