以临床发展需求为导向的医疗设备配置模式探讨

摘要 目的探索以临床发展需求为导向的医疗设备配置模式,以椎间孔镜设备为例,通过分析其配置与使用情况,为医疗设备管理提供科学依据。方法基于医疗设备台账数据及病案首页数据,回顾性分析2012至2023年21844例确诊为腰椎间盘突出症且行手术治疗患者的病案首页数据,对比传统手术和微创手术的手术量、住院日、手术时长及费用的变化趋势,探究手术方式的变迁及其过程中椎间孔镜增配情况的变化趋势。结果微创手术量及占比呈逐年增高趋势,且手术用时、平均住院日及费用均低于传统手术组(p<0.01)。微创手术使用的医疗设备椎间孔镜配置数量由1台增至8台,单机工作量平均在15例/月并趋于稳定,当单机工作量达到20例/月之后,微创手术量增加需要调整临床工作模式。结论相较于传统手术,微创手术在治疗腰椎间盘突出症方面具有显著优势。以临床需求为导向的医疗设备配置模式具有现实意义,能够有效指导设备决策,避免资源浪费。 Objective To explore a medical equipment configuration model guided by clinical development needs,using intervertebral foramen endoscopy equipment as an example.By analysing its configuration and utilisation patterns,this study aims to provide scientific evidence for medical equipment management.Methods Based on medical equipment ledger data and medical record header information,a retrospective analysis was conducted on 21,844 cases diagnosed with lumbar disc herniation undergoing surgical treatment between 2012 and 2023.Trends in surgical volume,length of hospital stay,operative duration,and costs were compared between traditional and minimally invasive procedures to investigate shifts in surgical techniques and the corresponding trends in intervertebral foramen endoscope provision.Results The volume and proportion of minimally invasive procedures showed a year-on-year increase,with significantly shorter operative times,shorter mean hospital stays,and lower costs compared to the traditional surgery group(p<0.01).The number of intervertebral foramen endoscopes deployed for minimally invasive surgery increased from one to eight units.The average workload per unit stabilised at approximately 15 cases per month.When the workload per unit exceeds 20 cases per month,adjustments to clinical workflows become necessary to accommodate further increases in minimally invasive procedures.Conclusion Minimally invasive surgery demonstrates significant advantages over traditional techniques in treating lumbar disc herniation.A clinically demand-driven approach to medical equipment configuration holds practical significance,effectively guiding procurement decisions and preventing resource wastage.
