摘要
目的 探讨壶腹周围癌患者胰十二指肠切除术后胃排空障碍发生的影响因素。方法 采用前瞻性研究。选取2018年5月至2023年7月河南省驻马店市中心医院肝胆胰脾外二科收治的61例拟行胰十二指肠切除术的壶腹周围癌患者作为研究对象,根据术后胃排空障碍发生情况将患者分为发生组(30例)与未发生组(31例),收集并比较2组患者的基线资料,采用单因素、多因素Logistic回归性分析检验影响壶腹周围癌患者胰十二指肠切除术后并发胃排空障碍的影响因素。结果 61例患者30例出现胃排空障碍,发生率49.18%(30/61)。2组在身体质量指数(BMI)、术式以及术后血糖方面比较,差异有统计学意义(P<0.05)。Logistic回归性分析检验显示BMI、术式以及术后血糖是壶腹周围癌患者胰十二指肠切除术后并发胃排空障碍的影响因素(P<0.05)。结论 壶腹周围癌患者胰十二指肠切除术后发生胃排空障碍的风险较高,BMI、术式以及术后血糖是其影响因素。
Objective To analyze the influencing factors of gastric emptying disorders in patients with periampullary carcinoma after pancreaticoduodenectomy.Methods A prospective study was conducted to select 61 patients with periampullary carcinoma who underwent pancreaticoduodenectomy.According to the occurrence of postoperative gastric emptying disorders,patients were divided into an occurrence group(30 cases)and a non occurrence group(31 cases).Baseline data of the 2 groups of patients were collected and compared.Univariate and multivariate Logistic regression analysis was used to test the influencing factors of gastric emptying disorders in patients with periampullary carcinoma after pancreaticoduodenectomy.Results Out of 61 patients,30 developed gastric emptying disorders,with an incidence rate of 49.18%(30/61).There was a statistical significant difference between the 2 groups in terms of body mass index(BMI),surgical procedures,and postoperative blood glucose(P<0.05).Logistic regression analysis showed that BMI,surgical procedure,and postoperative blood glucose were the influencing factors for postoperative gastric emptying disorders in patients with periampullary carcinoma after pancreaticoduodenectomy(P<0.05).Conclusion Patients with periampullary carcinoma have a higher risk of gastric emptying disorders after pancreaticoduodenectomy.BMI,surgical procedure,and postoperative blood glucose are factors that affect the occurrence of gastric emptying disorders and should be given clinical attention.
