摘要
目的基于健康行动过程取向理论模型,深入了解乳腺癌患者术后对参与上肢功能锻炼障碍的主观体验。方法采用目的抽样法,选取2024年1月—3月在山西白求恩医院乳腺外科住院的14例乳腺癌术后患者进行半结构式访谈,根据Colaizzi 7步分析法对访谈资料进行分析并提炼主题。结果根据健康行动过程取向理论模型的3个阶段将乳腺癌患者术后上肢功能锻炼障碍归纳为3个主题:意识行为冲突(风险感知能力薄弱、认知行为偏差);行为执行力弱化(恐惧运动、化疗相关的非自主性身体限制、责任及时间压力);社会支持缺乏(社会疏离感、信息材料及锻炼计划的适用性),3个主题相互关联、彼此影响。结论本研究揭示了乳腺癌术后患者坚持上肢功能锻炼的主要障碍。医护人员应从乳腺癌患者的角度出发,分析患者的个体差异,了解患者术后参与功能锻炼的主观感受、困惑及需求,帮助患者规避不良的认知,同时重视多种支持的来源,促进乳腺癌患者上肢功能锻炼行为的改善。
Objective Based on the theory of health action process approach model,this pa⁃per aims to deeply understand the subjective experience of breast cancer patients participating in upper limb functional exercise after operation.Methods A purposive sampling method was used to select 14 patients with breast cancer who were hospitalized in Shanxi Bethune Breast Surgery from January to March 2024 for semi-structured interviews.The interview data were analyzed and the theme was refined according to Colaizzi's 7-step analysis method.Results According to the three stages of the health action process approach theory model,the barriers to postoperative upper limb functional exercise in breast cancer patients was summarized into three themes:conscious⁃ness behavior conflict(weak risk perception ability,cognitive behavior deviation);weakening of behavioral execution(fear of exercise,chemotherapy-related non-autonomous physical limita⁃tions,responsibility and time pressure);lack of social support(social alienation,applicability of information materials and exercise programs),three themes are interrelated and influence each other.Conclusion This study reveals the main obstacles for breast cancer patients to adhere to upper limb functional exercise.From the perspective of breast cancer patients,medical staff should analyze the individual differences of patients,understand the subjective feelings,confu⁃sion and needs of patients participating in functional exercise after operation,help patients avoid bad cognition,and pay attention to various sources of support to promote the improvement of up⁃per limb functional exercise behavior of breast cancer patients.
