摘要
目的:探究牙周-正畸联合治疗对牙周炎致前牙移位患者血清碱性磷酸酶、骨钙素以及骨保护素水平的影响。方法:选择2022年5月~2024年5月我院接诊的54例牙周炎致错位前牙患者为研究对象,所有患者均给予牙周-正畸联合治疗,治疗前评估所有牙周炎致错位前牙患者血清碱性磷酸酶、骨钙素、骨保护素水平,术后连续随访12个月,评估其疗效。54例牙周炎致前牙移位患者治疗效果明显者将其纳入有效组(50例),无效者纳入无效组(4例)。对比两组血清碱性磷酸酶、骨钙素、骨保护素水平,采用Logistics回归分析血清碱性磷酸酶、骨钙素、骨保护素与牙周-证据联合治疗牙周炎致前牙移位患者效果的关系;分析血清碱性磷酸酶、骨钙素、骨保护素评估牙周-正畸联合治疗牙周炎致前牙移位患者效果的价值。结果:治疗前,两组血清碱性磷酸酶、骨钙素以及骨保护素水平比较,差异均无统计学意义(P>0.05);治疗后,两组各项指标水平高于治疗前,有效组各项指标水平均高于无效组(P<0.05)。将牙周-正畸联合治疗牙周炎致前牙移位患者的效果作为因变量(0=治疗无效,1=治疗有效),将血清碱性磷酸酶、骨钙素、骨保护素作为协变量,经Logistics回归分析显示,血清碱性磷酸酶、骨钙素、骨保护素水平与牙周炎致前牙移位患者疗效相关(P<0.05)。联合诊断评估牙周-正畸联合治疗牙周炎致前牙移位患者效果的AUC、约登指数、特异度及敏感度明显高于单项指标。结论:牙周-正畸联合治疗牙周炎致前牙移位患者,能够提高血清碱性磷酸酶、骨钙素、骨保护素水平,且疗效明显。
Objective:To explore the influence of combined periodontal-orthodontic treatment on the levels of serum alkaline phosphatase,osteocalcin and osteoprotegerin in patients with anterior tooth displacement caused by periodontitis.Methods:Fifty-four patients with displaced anterior teeth caused by periodontitis who were treated in our hospital from May 2022 to May 2024 were selected as the research subjects.All patients were given combined periodontal-orthodontic treatment.Before treatment,the levels of serum alkaline phosphatase,osteocalcin and osteoprotegerin in all patients with displaced anterior teeth caused by periodontitis were evaluated.The patients were followed up continuously for 12 months after the operation,the therapeutic effect was evaluated.Fifty-four patients with anterior tooth displacement caused by periodontitis who achieved significant therapeutic effects were included in the effective group(50 cases).Those with no therapeutic effect were included in the ineffective group(4 cases).The levels of serum alkaline phosphatase,osteocalcin and osteoprotegerin between the two groups were compared.Logistic regression was used to analyze the relationship between serum alkaline phosphatase,osteocalcin,osteoprotegerin and the effect of periodontitis-evidence combined treatment on patients with anterior tooth displacement caused by periodontitis.To analyze the value of serum alkaline phosphatase,osteocalcin and osteoprotegerin in evaluating the effect of combined periodontal-orthodontic treatment on patients with anterior tooth displacement caused by periodontitis.Results:Before treatment,there was no statistically significant difference in the levels of serum alkaline phosphatase,osteocalcin and osteoprotegerin between the two groups(P>0.05).After treatment,the levels of various indicators in both groups were higher than those before treatment,and those in the effective group were higher than in the ineffective group(P<0.05).The effect of combined periodontal-orthodontic treatment for patients with anterior tooth displacement caused by periodontitis was taken as the dependent variable(O=treatment ineffective,1=treatment effective).Serum alkaline phosphatase,osteocalcin and osteoprotegerin were taken as covariates.Logistic regression analysis shows that,the levels of serum alkaline phosphatase,osteocalcin and osteoprotegerin were correlated with the therapeutic effect of patients with anterior tooth displacement caused by periodontitis(P<0.05).The AUC,Youden index,specificity and sensitivity of the combined diagnosis for evaluating the effect of combined periodontal-orthodontic treatment on patients with anterior tooth displacement caused by periodontitis were significantly higher than single indicator.Conclusion:The combined periodontal-orthodontic treatment for patients with anterior tooth displacement caused by periodontitis can increase the levels of serum alkaline phosphatase,osteocalcin and osteoprotegerin,and the therapeutic effect is obvious.
