摘要
腓总神经从坐骨神经分出后,绕行于腓骨颈外侧,此处位置表浅且紧贴骨膜,易受压迫或外伤。腓总神经损伤的主要临床表现以足背伸障碍、足下垂、足内翻,以及外踝、足背外侧面的皮肤感觉障碍为主。腓总神经损伤可归属于中医学“痿证”或“经筋病”范畴,以患肢运动受限、肌肉萎缩、无力、动作迟缓以及筋脉松弛为特征。气血亏虚与脏腑经脉失养是导致肢体痿弱的重要原因。对于该病的治疗,曹玉霞主任医师倡导采用针灸优势技术组合,发挥“循经排刺+电针+特定电磁波谱(TDP)照射+穴位注射”的协同作用。其中循经排刺以足三阳经为核心,选取秩边、阳陵泉、足三里等关键穴位,直接刺激腓总神经分布区,改善局部气血运行;电针疗法通过微电流激活神经-体液调节机制,促进神经生长因子分泌,加速轴突再生速度;TDP照射通过特定电磁波的热效应,促使血管扩张,从而增加局部血流量,有助于消除炎性水肿;穴位注射维生素B1和维生素B12混合液,通过经络给药,增强神经营养与修复效率。该法通过融合多种技术的优势,起到疏通经络、平衡阴阳、强筋健骨、濡养筋脉等作用,从而促进受损神经恢复,实现治愈疾病的目的。
The common peroneal nerve branches from the sciatic nerve and courses superficially around the lateral aspect of the fibular neck,closely adherent to the periosteum,rendering it vulnerable to compression or trauma.Common peroneal nerve injury primarily manifests as dorsiflexion impairment,foot drop,foot inversion,and sensory disturbances over the lateral malleolus and dorsum.This condition corresponds to“flaccidity syndrome”or“muscle region disease”in traditional Chinese medicine,characterized by motor limitation,muscle atrophy,weakness,movement retardation,and tendon laxity in the affected limb.Qi-blood deficiency and malnourishment of visceral meridians constitute key pathogenic mechanisms.Chief Physician Cao Yuxia advocates a synergistic combination of acupuncture techniques:meridian-aligned cluster needling+electroacupuncture+specific electromagnetic spectrum irradiation(TDP)+acupoint injection.The meridianaligned cluster needling targets the three yang meridians of the foot,selecting key acupoints including Zhibian(BL54),Yanglingquan(GB34),and Zusanli(ST36)to directly stimulate the common peroneal nerve distribution area and improve local qi-blood circulation.Electroacupuncture employs microcurrent to activate neurohumoral regulation,promoting nerve growth factor secretion and accelerating axonal regeneration.TDP irradiation utilizes thermal effects of specific electromagnetic waves to induce vasodilation,thereby increasing local blood flow and resolving inflammatory edema.Acupoint injection of thiamine and cyanocobalamin mixtures enhances nerve nutrition and repair efficiency through meridian-based drug delivery.This integrated approach synergistically dredges meridians,balances yin-yang,strengthens tendons and bones,and nourishes sinews,ultimately promoting neural recovery and achieving therapeutic goals.
