摘要
溃疡性结肠炎(UC)是消化系统常见且难以治愈的慢性疑难疾病,其临床主要表现为反复发作的腹痛、腹泻、黏液脓血便等,内镜下可见黏膜炎症呈连续性分布,可累及直肠、结肠的不同节段,目前具体病因尚未完全明确,一般认为与免疫、遗传、环境及肠道微生态等多因素相关。在中医学理论指导下,通过辨证与辨病相结合的方法,对该病进行个体化处方用药,显示出独特优势。文章基于中医“三因制宜”原则,即因时、因地、因人制宜,提出“湿热损络致疡”作为溃疡性结肠炎的核心病机理论。该理论强调湿热内蕴、壅滞肠络,进而血败肉腐、内溃成疡的病理过程,高度概括了本病活动期以邪实为主的关键机制。文章系统阐述“湿热损络致疡”理论的内涵,并分别从溃疡性结肠炎活动期、活动与缓解交替期及缓解期3个阶段,深入剖析其动态演变的病机特点,进一步提出分期论治的中医辨治框架,为临床从“湿热损络”角度防治溃疡性结肠炎提供了重要的理论依据与实践指导。
Ulcerative colitis(UC)is a common yet refractory chronic digestive disease characterized clinically by recurrent abdominal pain,diarrhea,and mucopurulent bloody stools.Endoscopy reveals continuous mucosal inflammation that may affect various segments of the rectum and colon.Although its exact etiology remains unclear,it is generally associated with immune dysfunction,genetic predisposition,environmental factors,and gut microbiota imbalances.Guided by traditional Chinese medicine(TCM)theory,the integration of pattern identification and disease diagnosis has demonstrated distinct advantages in personalized treatment.Based on the TCM principle of treatment tailored to time,place,and person,propose"damp-heat impairing collaterals and inducing ulcers"as the core pathogenesis of UC.This theory emphasizes the pathological process wherein internal damp-heat stagnates in the intestinal collaterals,leading to tissue decay and ulcer formation,succinctly capturing the dominance of pathogenic excess in the active stage.This article systematically elaborates on the connotation of this theory,analyzes the dynamic pathological evolution across the active,alternating,and remission stages,and further establishes a staged TCM diagnostic and treatment framework,providing theoretical and practical guidance for managing UC from the perspective of"damp-heat impairing collaterals."
