摘要
目的:探究超早期急性期脑梗死(Acute cerebral infarct,ACI)磁共振血管成像(Magnetic Resonance Angiography,MRA)、磁共振弥散加权成像(Diffusion Weighted Imaging,DWI)特征及其联合诊断价值。方法:回顾性选取2023年3月~2025年3月我院收治的均接受MRA、DWI检查的62例超早期ACI患者。观察患者MRA、DWI图像特征,并分析其联合对超早期ACI的诊断价值。结果:62例超早期ACI患者经“金标准”检查结果发现,单发病灶23例,多发病灶29例。62例超早期ACI患者MRA图像均存在明显的血管异常,检出率为100%;62例超早期ACI患者DWI图像均为高信号,存在明显的血管异常,且ADC图像均为明显低信号,其主要病灶检出率及阳性检出率均为100%。MRA、DWI单独及其联合对超早期ACI患者的阳性检出率均为100%(62/62)显著高于常规颅脑MRI扫描的阳性检出率66.13%(41/62)(P<0.05)。结论:MRA和DWI单独或联合对超早期ACI均有极高的诊断价值,且还能明确患者病灶大小及位置等信息。
Objective:To explore magnetic resonance angiography(MRA)and diffusion-weighted imaging(DWI)features of ultra-early acute cerebral infarction(ACI)and diagnostic value of combination of the two.Methods:A total of 62 patients with ultra-early ACI who underwent MRA and DWI examinations in the hospital from March 2023 to March 2025 were reviewed.Their MRA and DWI image features were observed.The diagnostic value of MRA combined with DWI for ultraearly ACI was analyzed.Results:The"gold standard"examination results of the 62 patients with ultra-early ACI showed that 23 cases had single lesions and 29 cases had multiple lesions.MRA images of all patients showed significant vascular abnormalities,with a detection rate of 100%.DWI images of the patients all showed hyperintensity,with obvious vascular abnormalities.Moreover,the ADC images all showed significant hypointensity.The detection rate and positive detection rate of main lesions both were 100%.The positive detection rates of MRA,DWI,and combination of the two for ultra-early ACI all were 100%(62/62),significantly higher than that of conventional brain MRI[66.13%(41/62)](P<0.05).Conclusion:MRA,DWI,and combination of the two all exhibit high diagnostic value for ultra-early ACI.They can also help identify the size and location of the lesion.
