摘要
目的:将提升医务人员手卫生依从性措施的系统评价/Meta分析进行再评价。方法:计算机系统检索中国知网、万方数据库、中国生物医学文献数据库、维普数据库、PubMed、EMbase、CINAHL、the Cochrane Library,采用主题词与自由词相结合的方式系统检索,检索时限均为建库至2023年12月。由2名具有硕士研究生学历且接受循证医学系统培训的研究人员独立进行文献筛选和资料提取,并应用系统评价方法学质量评价工具2.0(AMSTAR 2)进行方法学质量评价后,采用推荐分级的评估、制订与评价系统进行证据的汇总与分级。结果:共纳入17篇系统评价,AMSTAR 2评价结果显示,4篇(23.53%)质量等级为高级;2篇(11.76%)质量等级为中级;3篇(14.65%)质量等级为低级;8篇(47.06%)质量等级为极低级。对纳入文献的10条证据进行质量评价,结果显示,6条证据质量为高级,3条证据为中级,1条证据为低级。结论:综合干预措施的效果可提高医务人员手卫生依从性;智能手卫生技术在医院中发挥着不可或缺的作用。但考虑到目前纳入系统评价的整体研究质量较低,未来仍需严格、规范开展高质量的研究,提供更有力的证据支持。
Objective:To evaluate the systematic review/Meta-analysis of interventions to improve hand hygiene compliance in healthcare workers.Methods:CNKI,WanFang Data,CBM,VIP,PubMed,EMbase,CINAHL,Cochrane Library were searched by computer to find systematic reviews/Meta-analysis of interventions to improve hand hygiene compliance in healthcare workers using a combination of subject terms and free words systematic searches,with the timeframe of all searches being from the establishment of the database to December 2023.Literature screening and data extraction were performed independently by two researchers with a master's degree and training in evidence-based medicine systems.AMSTAR 2.0 scale was used to evaluate methodological quality and GRADE system was used to summarize and grade the evidence.Results:A total of 17 systematic evaluations were included,and AMSTAR 2 evaluations showed that 4(23.53%)were rated as high quality,2(11.76%)were rated as medium quality,3(14.65%)were rated as low quality,and 8(47.06%)were rated as very low quality.A quality assessment of 10 pieces of evidence from the included literature showed that 6 were of high quality,3 were of medium quality,and 1 was of low quality.Conclusion:The effectiveness of comprehensive interventions can significantly improve hand hygiene compliance among healthcare workers,smart hand hygiene technology plays an integral role in hospitals.However,considering the low quality of the overall studies included in the systematic evaluation,there is still a need for rigorous and standardized high-quality studies to provide stronger evidence support in the future.
