摘要
目的:系统评价心力衰竭病人容量管理体验,为制订精准化干预策略提供依据。方法:计算机系统检索PubMed、Web of Science、EMbase、CINAHL、Cochrane Library、中国知网、万方数据库、维普数据库、中国生物医学文献数据库中与心力衰竭病人容量管理相关的质性研究,检索时限为建库至2025年2月10日。采用汇集性整合法进行Meta整合。结果:共纳入13篇文献,提炼出28个研究结果,归纳为10个新的类别,汇总成4个整合结果,分别为症状识别与认知障碍、心理社会适应障碍、知识-动机-执行困境、社会支持系统失能。结论:现有证据表明,心力衰竭病人容量管理现状呈现多维复杂性,未来可重点开发文化适配性教育工具与智能监测系统,以提升病人自我管理效能及生存质量。
Objective:Systematically evaluate volume management experiences in heart failure patients to inform precision intervention strategies.Methods:Computer system searched PubMed,Web of Science,EMbase,CINAHL,Cochrane Library,CNKI,WanFang Data,VIP,CBM for qualitative studies related to volume management of patients with heart failure.The retrieval time was from the inception to February 10,2025.Using a summative integration approach to perform a Meta-integration.Results:A total of 13 articles were included,yielding 28 findings categorized into 10 themes and synthesized into 4 key results,which were symptom recognition/cognitive deficits,psychosocial adaptation challenges,knowledge-motivation-action gaps,and social support system inadequacies.Conclusion:Current evidence shows that volume management in heart failure patients is complex.Future work should focus on developing culturally adapted educational tools and intelligent monitoring systems to enhance self-management efficacy and quality of life.
