腺病毒感染患儿病情严重程度与血清乳酸脱氢酶、C反应蛋白水平的相关性研究

摘要 目的探讨腺病毒感染患儿病情严重程度与血清乳酸脱氢酶(LDH)、C反应蛋白(CRP)水平的相关性。方法选取安徽省儿童医院2023年4月至2024年3月腺病毒感染患儿112例作为研究对象,根据疾病严重程度将其分为轻症组(n=67)和重症组(n=45),分析所有患儿临床资料,采用Logistic分析导致腺病毒感染患儿病情加重的影响因素,Kendall's tau-b分析腺病毒感染患儿病情严重程度与血清LDH、CRP水平的相关性,绘制受试者工作特征(ROC)曲线分析血清LDH、CRP水平对腺病毒感染患儿病情严重程度的预测价值。结果重症组患儿LDH、CRP高于轻症组(P<0.05);Logistic分析显示,血清LDH、CRP水平是腺病毒感染患儿病情加重的影响因素(P<0.05);采用Kendall's tau-b分析发现,血清LDH、CRP与腺病毒感染患儿病情严重程度均呈正相关(相关系数=0.551,0.633;P<0.001);血清LDH、CRP在腺病毒感染患儿病情严重程度的ROC曲线下面积(AUC)分别为0.895、0.952,对应灵敏度为89.90%、93.30%,特异度为92.50%、94.00%,二者联合AUC为0.970,灵敏度和特异度分别为93.30%和97.00%,Hosmer-Lemeshow结果显示,χ^(2)=6.230,P=0.621,AUC均>0.7,说明模型诊断效能高。结论随着腺病毒感染患儿病情加重,血清LDH、CRP水平明显上升,监测LDH、CRP水平变化对患儿的治疗有一定指导意义。 Objective To investigate the association between disease severity and serum lactate dehydrogenase(LDH)and C-reactive protein(CRP)levels in children with adenovirus infection.Methods A total of 112 children with adenovirus infection admitted to Anhui Provincial Children's Hospital from April 2023 to March 2024 were enrolled and categorized into mild(n=67)and severe(n=45)groups based on their disease severity.The clinical data were analyzed.Logistic regression analysis was performed to identify risk factors for disease progression.Kendall's tau-b correlation analysis was utilized to evaluate the correlation between disease severity and serum LDH and CRP levels.Receiver operating characteristic(ROC)curves were plotted to assess the predictive value of serum LDH and CRP levels for disease severity.Results Serum LDH and CRP levels in the severe group were higher than those in the mild group(P<0.05).Logistic regression analysis identified serum LDH and CRP levels as independent risk factors for disease progression in children with adenovirus infection.Kendall's tau-b analysis revealed that serum LDH and CRP were positively correlated with the severity of adenovirus infection in children(correlation coefficient=0.551,0.633;P<0.001).The areas under the ROC curve(AUCs)of serum LDH and CRP in predicting the severity of adenovirus infection in children were 0.895 and 0.952,with the corresponding sensitivity of 89.90%and 93.30%,and specificity of 92.50%and 94.00%,respectively.The AUC of the combination of serum LDH and CRP was 0.970,with a sensitivity of 93.30%and a specificity of 97.00%.The Hosmer-Lemeshow results confirmed the high diagnostic efficacy of the model(χ^(2)=6.230,P=0.621,and AUCs>0.7).Conclusion Serum LDH and CRP levels significantly increase with disease progression in children with adenovirus infection.Monitoring the two biomarkers may guide clinical management and risk stratification.
