摘要
目的:构建并实施处方前置审核全面质量管理(TQM)体系,提升审核质量与药事服务水平。方法:基于TQM理念,系统优化审方系统集成性能,确保审核规则科学性及规范标准化审核流程,并建立多维度质量评价体系。结果:TQM实施后,系统规则库得到有效维护(共11077条),审核质量显著提升:门诊与住院任务合格率分别提高至99.02%和95.40%(P<0.01);系统拦截率分别升至1.95%和0.62%(P<0.01);药师审核环节的拒绝发药率显著增长(门诊17.57%,住院11.40%,P<0.01),平均审核时间缩短,医生对审核结果的依从性也明显改善。结论:TQM体系可有效提升处方前置审核质量与效率,有利于医院合理用药管理和医疗质量改进。
Objective:To construct and implement a total quality management(TQM)system for prescription pre-review to enhance audit quality and the level of pharmaceutical services.Methods:Based on the TQM philosophy,we systematically optimized the integration performance of the prescription review system,ensured the scientificity of review rules,standardized the review process,and established a multidimensional quality evaluation system.Results:After the implementation of TQM,a total of 11077 system rules were maintained,and the quality of prescription review was significantly improved:The pass rates for outpatient and inpatient review tasks increased to 99.02%and 95.40%,respectively(P<0.01);the system interception rates rose to 1.95%and 0.62%for outpatient and inpatient cases,respectively(P<0.01);the rates of drug dispensing refusal during the pharmacist review process markedly increased(17.57%for outpatient and 11.40%for inpatient,P<0.01),while the average review time was shortened,and physicians’compliance with review results was also notably improved.Conclusion:The TQM system can effectively enhance the quality and efficiency of prescription pre-review,facilitating rational drug use management and medical quality improvement in hospitals.
