摘要
目的:探讨电针“骶四穴”联合常规治疗对脑卒中后神经源性膀胱(NB)患者排尿功能、残余尿及生活质量的影响。方法:随机数字表法结合隐蔽分组60例患者分为对照组(30例,研究组30例。对照组常规间歇导尿+膀胱功能训练)。2组均治疗1次/d,每周治疗5日,共4周。通过比较国际下尿路症状量表(LUTS)、残余尿指标及生活质量量表(QOL)评估疗效。结果:研究组总有效率(93.33%)显著高于对照组(70.00%)(P<0.05)。治疗后研究组LUTS评分降低幅度及QOL评分提升幅度均优于对照组(均P<0.05)。残余尿指标中,研究组改善显著(均P<0.05)。结论:电针“骶四穴”联合常规治疗可改善脑卒中NB患者储尿与排尿功能,缓解临床症状,具有优化生活质量的临床应用价值。
Objective:To explore the effects of electroacupuncture at“Sacro-Four Acupoints”combined with rou-tine treatment on urinary function,residual urine volume and quality of life in patients with neurogenic bladder(NB)after stroke.Methods:A total of 60 patients were divided into a control group and a study group by random number table method combined with concealed allocation.The control group was treated with routine intermittent catheteriza-tion plus bladder function training,while the study group was treated with the same therapy as the control group plus electroacupuncture at“Sacro-Four Acupoints”(bilateral Zhibian[BL 54]and Huiyang[BL 55]were selected;elec-troacupuncture parameters:continuous wave,pulse width of 0.2 ms,frequency of 2 Hz,and intensity based on patient tolerance).Both groups received treatment once a day,5 days a week,for a total of 4 weeks.The efficacy was evalu-ated by comparing the International Prostate Symptom Score(IPSS,also known as the International Lower Urinary Tract Symptom Scale[LUTS]),residual urine volume indicators and Quality of Life Scale(QOL).Results:The total effective rate of the study group(93.33%)was significantly higher than that of the control group(70.00%)(P<0.05).After treatment,the reduction range of LUTS score and the increase range of QOL score in the study group were better than those in the control group(both P<0.05).Among the residual urine volume indicators,the improvement in the study group was significant(all P<0.05).Conclusion:Electroacupuncture at“Sacro-Four Acupoints”combined with routine treatment can improve the urine storage and urinary function of stroke patients with NB,relieve clinical symp-toms,and has clinical application value in optimizing the quality of life.
