摘要
目的观察针刺结合温阳补肾灸治疗脑卒中后肾阳虚型尿失禁的临床疗效。方法将72例脑卒中后肾阳虚型尿失禁患者随机分为观察组和对照组,各36例。对照组予以针刺治疗,观察组在针刺基础上结合温阳补肾灸治疗,1次/d,每周6次,共治疗4周。观察比较两组患者治疗前后平均白天排尿次数、夜间排尿次数、每次尿量、尿常规白细胞计数、膀胱残余尿量、肾阳虚症状评分及尿失禁生活质量量表(I-QOL)评分。结果与治疗前比较,治疗后两组平均白天排尿次数、夜间排尿次数、白细胞计数及膀胱残余尿量均不同程度减少,肾阳虚症状改善,而单次平均尿量及I-QOL增加(P<0.01);与对照组比较,治疗后观察组平均白天排尿次数、夜间排尿次数、白细胞计数及膀胱残余尿量均显著减少(P<0.05或P<0.01),肾阳虚症状明显改善(P<0.05),而单次平均尿量及I-QOL显著增加(P<0.05或P<0.01)。结论针刺结合温阳补肾灸可以有效改善脑卒中后肾阳虚型患者的尿失禁症状,减少膀胱残余尿量,提高患者生活质量。
Objective To observe the clinical effect of acupuncture combined with warming Yang and tonifying kidney moxibustion on urinary incontinence of kidney Yang deficiency after stroke.Methods Seventy-two patients with urinary incontinence due to deficiency of kidney Yang after stroke were randomly divided into observation group and control group,with 36 cases in each.The control group was treated with acupuncture,and the observation group was treated with warming Yang and tonifying kidney moxibustion on the basis of acupuncture once a day,six times a week,a total of 4 weeks.Before and after treatment,the average daytime urination times,night urination times,each urine volume,urine routine white blood cell count,bladder residual urine volume,kidney Yang deficiency symptoms score and urinary incontinence quality of life scale(I-QOL)score of two groups were compared.Results Compared with those before treatment,the average times of urination during the day,times of urination at night,white blood cell count and bladder residual urine volume in the two groups were decreased to different degrees after treatment,and the symptoms of kidney Yang deficiency were improved,while the average urine volume and I-QOL were increased(P<0.01).Compared with those of the control group,the observation group significantly reduced the average number of urination during the day,the number of urination at night,the white blood cell count and the bladder residual urine volume after treatment(P<0.05 or P<0.01),and the symptoms of kidney Yang deficiency were significantly improved(P<0.05),while the single average urine volume and I-QOL were significantly increased(P<0.05 or P<0.01).Conclusion Acupuncture combined with warming Yang and tonifying kidney moxibustion can effectively improve the symptoms of urinary incontinence in patients with kidney Yang deficiency after stroke,reduce bladder residual urine volume,and improve patients’quality of life.
