摘要
间质性肺疾病急性加重(AE-ILD)是呼吸系统急危重症之一,病死率较高。王玉光教授通过长期的临床实践,总结出AE-ILD的主要病机及治法方药,在临床应用中取得了很好的疗效。王玉光教授认为AE-ILD在中医学属“喘脱”范畴,病位累及肺、脾、肾及三焦焦膜,主要责之于肾,主要病机为本虚标实,真元亏损欲脱为本、邪气郁闭肺气为标。寒邪郁闭肺气,气不能布散津液,则化生湿浊痰饮。证属元气虚衰将脱,寒邪凝滞收引,湿浊痰饮内蕴阻滞。治疗以回阳救逆、纳气固脱、散寒化湿为主;处方选用四逆汤合来复汤加减。
Acute exacerbation of interstitial lung disease(AE-ILD)is one of the critical and severe respiratory conditions with a high mortality rate.Professor WANG Yuguang,through extensive clinical practice,has summarized the main pathogenesis and treatment methods for AE-ILD,achieving remarkable therapeutic results in clinical applications.Professor WANG believes that AE-ILD,in terms of traditional Chinese medicine(TCM),belongs to the category of"asthma collapse".The disease primarily involves the lungs,spleen,kidneys,and the triple energizer membrane,with the kidneys being the primary organ responsible.The main pathogenesis is characterized by a deficiency at the root and an excess at the manifestation.The deficiency of true primordial qi leads to a tendency for collapse,while pathogenic qi obstructs lung qi.Cold pathogens constrain lung qi,preventing its dispersal,which results in the transformation of body fluids into dampness,turbidity,and phlegm.The syndrome is characterized by the decline of primordial qi leading to collapse,the stagnation and accumulation of cold pathogens resulting in contraction,and the internal obstruction of dampness,turbidity,and phlegm triggering blockage.The treatment mainly focuses on restoring yang and replenishing qi to prevent collapse,dispersing cold,and transforming dampness.The prescription chosen is a modified combination of Sini Decoction and Laifu Decoction.
