摘要
目的收集吴佩衡、戴丽三、姚贞白、康诚之四位滇南医学代表医师治疗小儿急惊风的医案处方,通过数据挖掘方法探索滇南医家治疗小儿急惊风的用药规律,为中医临床治疗小儿急惊风提供参考。方法通过收集四位滇南医学代表医师治疗小儿急惊风的医案处方,构建治疗小儿急惊风的医案处方数据库,汇总的数据进行标准化处理后导入古今医案云平台(V2.4.3)进行综合统计分析。结果通过文献数据挖掘和筛选,最终纳入数据库的医案处方有51首,中药125味,使用频次较高的药物为甘草、钩藤和白芍;药性主要以清热解毒、补脾益气为主;药物四气以平性、寒性和温性为主;药物五味以甘、辛和苦为主;药物归经多归于肺、脾和心经。药物关联分析中排名前三的药对分别为白芍-甘草、钩藤-甘草、钩藤-天麻;药物聚类分析共分为4类。结论滇南医家治疗小儿急惊风药物处方以清热解毒、祛痰为主,兼顾平肝抑阳、镇惊止痉、补脾益肾,具有明辨虚实、表里兼顾、注重脏腑的特点。
Objective This paper aimed to explore the mentality and medication characteristics of the southern Yunnan physicians in treating pediatric acute convulsion by literature data mining.The data were collected from the medical records and prescriptions of representative physicians in southern Yunnan,such as Wu Peiheng,Dai Lisan,Yao Zhenbai,Kang Chengzhi,and so on.Methods The medical records and prescriptions for the treatment of pediatric acute convulsion were selected and constructed a database.After that,the data were standardized according to certain rules.And then the standardized data were imported into the Ancient and Modern Medical Record Cloud Platform(v2.4.3)for comprehensive statistical analysis.Results There were a total of 51 prescriptions and 125 traditional Chinese medicines(TCM)were ultimately selected from the database according to data statistical analysis.The highest frequently used TCM were Glycyrrhiza uralensis Fisch.,Uncaria rhynchophylla(Miq.)Miq.ex Havil.,and Paeonia lactiflora Pall.after statistical analysis.In these prescriptions,the efficacy of TCM were used more frequently such as clearing heat and detoxify,invigorating the spleen and replenishing‘Qi’.The‘Si Qi’of TCM were mainly characterized by steady,coldness and warmth.The‘wu wei’of TCM were mainly sweet,pungent and bitter.The meridian tropism of TCM mainly attributed to the lung,spleen and heart.The most frequently occurring drug pairs in TCM association analysis were Paeoniae Radix Alba-Radix Rhizoma Glycyrrhizae,Uncariae Ramulus Cum Uncis-Radix Rhizoma Glycyrrhizae,Uncariae Ramulus Cum Uncis-Gastrodiae Rhizoma.In addition,the TCM for treating pediatric acute convulsion was divided into four categories by data cluster analysis.Conclusion The medication characteristics of the southern Yunnan physician in treating pediatric acute convulsion was implement by clearing heat,detoxification and expectorant.Simultaneously,the physicians also equally emphasized treatment by regulating‘ping gan yi yang’,‘zhen jing zhi jing’,and‘bu pi yi shen’etc.
