摘要
目的探讨低切迹锁定小钢板手术治疗肱骨外科颈骨折合并肩关节脱位的效果及对患者关节功能的影响。方法回顾性分析2023年3月至2024年2月上饶市人民医院收治的62例肱骨外科颈骨折合并肩关节脱位患者的临床资料,按照治疗方式不同为观察组、对照组,各31例。对照组给予肱骨近端钢板内固定治疗,观察组给予低切迹锁定小钢板手术治疗。比较两组手术效果及复位情况;比较两组并发症情况;比较两组术后肩关节功能情况。结果观察组术中出血量少于对照组,手术时间、住院时间、骨愈合时间短于对照组,差异有统计学意义(P<0.05)。观察组的并发症总发生率低于对照组,差异有统计学意义(P<0.05)。观察组骨折复位优良率高于对照组,差异有统计学意义(P<0.05)。术后3个月,观察组肩关节功能各维度评分与总分均高于对照组,差异有统计学意义(P<0.05)。结论低切迹锁定小钢板手术治疗肱骨外科颈骨折合并肩关节脱位,创伤少,加速骨折端愈合,复位成功率高,术后肩关节功能恢复好,且安全可靠。
Objective To explore the effect of low-profile locking mini-plate surgery in the treatment of humeral surgical neck fracture with shoulder dislocation and its influence on joint function.Methods The clinical data of 62 patients with humeral surgical neck fracture with shoulder dislocation admitted to Shangrao People's Hospital from March 2023 to February 2024 were retrospectively analyzed.According to different treatment methods,they were divided into observation group and control group,with 31 cases in each group.The control group was treated with proximal humerus plate internal fixation,and the observation group was treated with low-notch locking mini-plate surgery.The surgical effect and reduction were compared between the two groups.The complications of the two groups were compared.The postoperative shoulder function was compared between the two groups.Results The intraoperative blood loss of the observation group was less than that of the control group,and the operation time,hospitalization time,and bone healing time were shorter than those of the control group,and the differences were statistically significant(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).The excellent rate of fracture reduction in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Three months after operation,the scores of each dimension and the total score of shoulder joint function in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion The treatment of humeral surgical neck fracture combined with shoulder dislocation with low-notch locking mini-plate has less trauma,accelerates fracture healing,high success rate of reduction,and good recovery of shoulder joint function after operation,and is safe and reliable.
