摘要
目的 探讨睿米机器人辅助接触性内窥镜技术在颅内病变活检术中应用的安全性和有效性。方法 回顾性分析2023年11月至2024年1月采用颅内病变活检术处理的16例患者的临床资料。术前行头部CT及MRI T_1三维磁化强度预备梯度回波序列扫描,将CT和MRI数据导入睿米机器人工作站,设计穿刺路径;术中采用接触性内窥镜肉眼下观察穿刺过程,避开穿刺路径上的血管,分辨靶点病变组织和正常脑组织,取病变组织进行病理检查以明确诊断。结果 16例均顺利完成活检术。术后病理诊断:弥漫星形细胞瘤和少突胶质细胞肿瘤7例,原发中枢神经系统弥漫大B淋巴瘤6例,颅咽管瘤1例,脑结核瘤1例,弥漫性脑膜黑色素细胞增生症1例。活检阳性率为100%(16/16)。术后无病灶出血和无穿刺路径出血。结论 睿米机器人辅助接触性内窥镜技术可以明显提高颅内病变活检术的安全性和有效性,减少术后并发症。
Objective To evaluate the safety and efficacy of the Remebot robotic-assisted contact endoscopic technique for biopsy of intracranial lesions.Methods A retrospective analysis was conducted on the clinical data of 16 patients who underwent intracranial lesion biopsy between November 2023 and January 2024.Preoperative head CT and MRI T1-weighted 3D magnetization-prepared rapid gradient-echo sequences were performed.The CT and MRI data were imported into the Remebot robotic workstation to plan the puncture path.During surgery,the puncture procedure was visually observed under a contact endoscope to avoid blood vessels along the puncture path,distinguish target lesion tissue from normal brain tissue,and collect lesion samples for pathological examination to confirm the diagnosis.Results All 16 biopsies were successfully completed.Postoperative pathological diagnoses included 7 cases of diffuse astrocytoma and oligodendroglial tumor,6 cases of primary central nervous system diffuse large B-cell lymphoma,1 case of craniopharyngioma,1 case of cerebral tuberculoma,and 1 case of diffuse meningeal melanocytosis.The biopsy positive rate was 100%(16/16).No postoperative hemorrhage at the lesion site or along the puncture path was observed.Conclusion The Remebot robotic-assisted contact endoscopic technique can significantly improve the safety and efficacy of intracranial lesion biopsy and reduce postoperative complications.
