摘要
目的:观察通络止痹方联合康复功能训练对全膝关节置换术后患者下肢功能及炎性指标的影响。方法:将80例全膝关节置换术后患者随机分为观察组(n=40)和对照组(n=40)。两组均接受相同康复功能训练,观察组加用通络止痹方,治疗2周。治疗前后评估两组疼痛数字评分(NRS)、患侧与健侧肢体周径差、膝关节活动度(ROM)、美国特种外科医院(HSS)评分、“起立-行走”计时测试(TUGT)评分、10 m步行时间及血清C反应蛋白(CRP)、血沉(ESR)水平,统计临床疗效与不良反应。结果:观察组总有效率95.0%,高于对照组的77.5%(P<0.05)。治疗后,观察组NRS评分、患健侧周径差、TUGT评分、10 m步行时间及CRP、ESR水平均低于对照组,ROM和HSS评分高于对照组(均P<0.05)。两组均未发生明显不良反应。结论:通络止痹方联合康复训练可有效缓解全膝关节置换术后术后肿痛,改善膝关节功能与步行能力,抑制炎症反应,安全性良好。
Objective:To observe the effect of Tongluo Zhibi formula combined with rehabilitation function training on lower limb function and inflammatory indicators in patients after total knee arthroplasty(TKA).Methods:Eighty patients after TKA were randomly divided into an observation group(n=40)and a control group(n=40).Both groups received the same rehabilitation function training,while the observation group was additionally treated with Tongluo Zhibi formula for 2 weeks.Before and after treatment,the numerical rating scale(NRS)score,limb circumference difference between the affected and healthy sides,range of motion(ROM)of the knee,hospital for special surgery(HSS)score,timed up and go test(TUGT)score,10-meter walking time,and serum levels of C-reactive protein(CRP)and erythrocyte sedimentation rate(ESR)were evaluated.Clinical efficacy and adverse reactions were recorded.Results:The total effective rate in the observation group was significantly higher than that in the control group(95.0%vs 77.5%,P<0.05).After treatment,the observation group showed significantly lower NRS scores,affected-to-healthy limb circumference differences,TUGT scores,10-meter walking times and CRP and ESR levels compared with the control group,while ROM and HSS scores were significantly higher(all P<0.05).No significant adverse reactions were observed in either group.Conclusion:The combination therapy can effectively alleviate pain and swelling,improve knee joint function and walking ability,suppress inflammatory responses with good safety in patients after TKA.
