摘要
儿童腺样体肥大(AH)发病率呈上升趋势,严重影响患儿身心健康。现代医学认为,腺样体微炎症状态是其病理性增生的关键环节,巨噬细胞极化在其中发挥重要作用。“伏毒”作为一种特殊的致病因素,为解析儿童AH的发病机制提供新视角。本文从“伏毒-巨噬细胞极化-微炎症状态”路径探讨儿童AH的中医辨证论治,以解毒透邪、调节巨噬细胞极化、消除微炎症状态为原则,配合清热解毒、化痰散结、活血化瘀、益气扶正等治法,急性期用银翘散合苍耳子散加减;缓解期用二陈汤合桃红四物汤加减;稳定期用六君子汤合玉屏风散加减。以期为中医药防治儿童AH提供新的研究方向。
The incidence rate of adenoid hypertrophy(AH)in children is on the rise,seriously affects physical and mental health of children.Modern medicine believes that microinflammatory state of adenoids is a key link in their pathological proliferation,and macrophage polarization plays an important role in it.Vasotoxin as a special pathogenic factor,provides a new perspective for analyzing the pathogenesis of AH in children.This article explores treatment based on syndrome differentiation of traditional Chinese medicine in AH of children from the pathway of“vasotoxin-macrophage polarization-microinflammatory state”,based on the principles of detoxification,regulating macrophage polarization,and eliminating microinflammatory state,combined with treatment methods such as clearing heat and detoxification,resolving phlegm and nodules,promoting blood circulation and removing blood stasis,and nourishing qi and supporting the body,in the acute phase,use Yinqiao Powder combined with Canger Zi Powder with modification;in the relief period,use Erchen Decoction combined with Taohong Siwu Decoction with modification;in the stable period,use Liujunzi Decoction combined with Yupingfeng Powder with modification.In order to provide new research direction for traditional Chinese medicine and pharmacy in the prevention and treatment of AH in children.
