摘要
糖尿病是一种以慢性高血糖为主要特征的临床综合征,发病率在全球范围内持续上升。糖尿病的危害不仅在于血糖异常本身,随着病情发展,还可引发多种微血管和大血管并发症,大幅降低患者生活质量甚至危及生命安全。糖尿病视网膜病变(DR)是糖尿病最具代表性的微血管并发症之一,现已成为威胁糖尿病患者视力的首要原因和主要致盲性眼病。因此,对DR患者实施积极有效的临床干预至关重要。在DR复杂的发病机制中,血管内皮生长因子(VEGF)扮演着核心角色。VEGF是一种强效的多功能细胞因子,具有驱动新生血管形成及增加血管通透性的作用。基于VEGF在DR病理过程中的关键作用,抗VEGF疗法应运而生,并成为DR治疗的重要策略。该疗法通过特异性中和VEGF能够有效抑制病理性新生血管的生成、阻断异常VEGF信号通路、显著降低血管通透性、减轻相关炎症反应。故本文旨在综述抗VEGF治疗DR的临床应用进展,分别从DR发病机制、VEGF与DR的发病研究、抗VEGF治疗等方面进行探究,为抗VEGF治疗DR提供可靠的参考依据。
Diabetes mellitus is a clinical syndrome characterized by chronic hyperglycemia,and its incidence continues to rise worldwide.The harm of diabetes not only lies in the abnormal blood glucose itself,but also can cause a variety of microvascular and macrovascular complications with the development of the disease,which greatly reduces the quality of life of patients and even endangers life safety.Diabetic retinopathy(DR)is one of the most representative microvascular complications of diabetes,and has become the primary cause that threatens the vision of diabetic patients and the main cause of blinding eye disease.Therefore,it is essential to implement active and effective clinical intervention for DR patients.VEGF is a potent multifunctional cytokine that drives neovascularization and increases vascular permeability.Based on the key role of VEGF in the pathological process of DR,anti-VEGF therapy came into being and became an important strategy for DR treatment.This therapy can effectively inhibit the formation of pathological neovascularization,block abnormal VEGF signaling pathway,significantly reduce vascular permeability and reduce the related inflammatory response by specifically neutralizing VEGF.Therefore,this article aims to review the clinical application progress of anti-VEGF in the treatment of DR,and to explore the pathogenesis of DR,the pathogenesis of VEGF and DR,and anti-VEGF treatment,so as to provide a reliable reference for anti VEGF treatment of DR.
