摘要
目的探讨双重血浆分子吸附系统在脓毒症肝衰竭患者中的应用价值。方法选取脓毒症肝衰竭患者120例,根据治疗方案分为观察组(65例)和对照组(55例),两组均接受连续性静脉-静脉血液滤过,其中观察组加以双重血浆分子吸附系统治疗,观察两组治疗前后肝肾功能、凝血功能、序贯器官衰竭(SOFA)评分、急性生理学和慢性健康状况评分(APACHEⅡ评)、炎症因子差异。结果观察组治疗72 h后丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、总胆红素(TBIL)、血尿素氮(BUN)和血肌酐(Scr)低于对照组(P<0.05)。观察组治疗72 h后血小板计数(PLT)低于对照组(P<0.05),而凝血酶原活动度(PTA)和血小板聚集功能(PAG)高于对照组(P<0.05)。观察组治疗72 h后SOFA评分和APACHEⅡ评分低于对照组(P<0.05)。观察组治疗72 h后C反应蛋白(CRP)、白细胞介素(IL-6)和肿瘤坏死因子-α(TNF-α)和可溶性白细胞分化抗原14亚型(sCD14-ST)低于对照组(P<0.05)。结论双重血浆分子吸附系统在脓毒症肝衰竭患者治疗中有较好的应用价值,可改善患者凝血功能、肝肾功能及炎症水平。
Objective To explore the application value of dual plasma molecular adsorption system in patients with sepsis induced liver failure.Methods A total of 120 patients with sepsis induced liver failure were selected and divided into observation group(65 cases)and control group(55 cases)according to the treatment plan.Both groups were given continuous veno-venous hemofiltration,and the observation group was treated with a dual plasma molecular adsorption system.The differences in liver and kidney function,coagulation function,sequential organ failure(SOFA)score,acute physiology and chronic health status score(APACHE II),inflammatory factors between the two groups before and after treatment were observed.Results After 72 hours of treatment,the levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TBIL),blood urea nitrogen(BUN),and serum creatinine(Scr)in the observation group were lower than those in the control group(P<0.05).After 72 hours of treatment,the platelet count(PLT)of the observation group was lower than that of the control group(P<0.05),while the prothrombin activity(PTA)and platelet aggregation function(PAG)were higher than those of the control group(P<0.05).After 72 hours of treatment,the SOFA score and APACHE II score of the observation group were lower than those of the control group(P<0.05).After 72 hours of treatment,the levels of C-reactive protein(CRP),interleukin(IL-6),and tumor necrosis factor-α(TNF-α)and soluble leukocyte differentiation antigen 14 subtype(sCD14-ST)in the observation group were lower than those in the control group(P<0.05).Conclusion The dual plasma molecular adsorption system has good application value in the treatment of sepsis induced liver failure patients,which can improve their coagulation function,liver and kidney function,and inflammation levels.
