摘要
本文报道1例因口服含乌头碱成分的复方荆芥熏洗剂导致室性心动过速、乳酸性酸中毒、低钾血症及心肌损害的患者,入院时心电图显示室性心动过速,血气分析提示乳酸性酸中毒及低钾血症,血液毒检检测到乌头碱及苦参碱,给予气管插管、呼吸机辅助呼吸、洗胃、导泻、药用炭片吸附及药物治疗等综合治疗,结合血液灌流清除毒物及ECMO辅助治疗,患者心律失常得到控制,恢复窦性心律,神志清楚,成功拔除气管插管并停止ECMO辅助,巩固治疗后康复出院,提示及时准确的诊断与综合治疗对急性药物中毒患者至关重要。
This paper reported a case of ventricular tachycardia,lactic acidosis,hypokalemia and myocardial damage caused by oral administration of compound Jingjie fumigation and washing agent.Upon admission,electrocardiogram revealed ventricular tachycardia,and blood gas analysis indicated lactic acidosis and hypokalemia,and the blood toxicology test detected aconitine and matrine.Following comprehensive treatment,including tracheal intubation,ventilator-assisted breathing,gastric lavage,catharsis,adsorption with medicinal charcoal tablets,and pharmacological therapy,combined with hemoperfusion for toxin removal and extracorporeal membrane oxygenation(ECMO)as adjunctive therapy,the patient's arrhythmia was controlled,and the patient returned to sinus rhythm with clear consciousness.The tracheal tube was successfully removed,and ECMO support was discontinued.After consolidation treatment,the patient recovered and was discharged.This case highlights the critical importance of timely and accurate diagnosis and comprehensive treatment for patients with acute drug poisoning.
